POSITION: Registered Nurse Responsible to: Executive Director

General Description of Responsibilities

The Registered Nurse provides health screening and health monitoring functions in the Center. She/he reports to the Executive Director and may be responsible for management of the Center in the absence of the Director and Assistant Director.

Typical duties include but are not limited to:

1. Conducting intake interviews and performing initial assessment utilizing the UAI (Universal Assessment Instrument).
2. Administering medications.
3. Observing participants for adverse reactions to medication.
4. Providing in-service training as needed.
5. Monitoring participantsgeneral physical and mental health and functional capability. Discussing significant changes with the participant, family, or other responsible party, and, as needed, the participants physician or physicians nurse.
6. Manages the delivery of all medical services to ensure that needed services are provided in a timely manner by appropriate personnel. Monitors care given by other staff members (monthly vitals, weights).
7. Leading care planning conferences and developing interdisciplinary plans of care.
8. Providing health education for participants and their families as needed.
9. Teaching the staff and monitoring their ability to perform a limited number of special procedures that may be part of in-home/community-based family caregiving situations. These procedures may include such items as monitoring vital signs, simple dressing changes, medication administration, and catheter irrigations.
10. Making referrals to other agencies/organizations as needed.
11. Responding to emergencies in participant care.
12. Developing nursing procedures and protocols with the approval of the Director and the Board of Directors.
13. Reviewing and assessing charts of daily meal consumption and fluid intake by participants.
14. Documenting consistently in participant charts regarding health status and activity function.
15. Assuring care plan reviews are completed on time, in accordance with licensing regulations.
16. Documenting and reporting additionally for participants served under Medicaid Waiver or Veterans Administration authorization, and for long term care insurance reimbursement as needed.
17. Attending staff meetings.
18. Being available daily to meet families and discuss the days events as needed.

Knowledge, Skills, and Abilities

Ability to work well with a frail and/or impaired population. Excellent oral and written communication skills with strong background in health services. Strong interpersonal skills are paramount in effectively interacting with participants, families, and other staff members on a daily basis. Ability to apply knowledge to formulate, develop, implement and evaluate individualized plans of care.

Education and Experience

Current Virginia license as a Registered Nurse BSN preferred Three years experience in a related health care setting preferred. Demonstrated competence in working with the impaired elderly, or younger adults with early onset Alzheimers Disease or similar dementia. Strong knowledge of their physical, social and mental health needs. CPR certification Supervisory and planning experience preferred.

OTHER INFORMATION The Adult Care Center is a private, non-profit adult day health care located in Winchester and serving Winchester and the surrounding areas. The Center is licensed by the Virginia Department of Social Services and is a Medicaid, and Veterans Administration, Provider.
