Essential Role:Affect improvements in the overall health of individuals seeking Community Cares services by providing professional care management & coordination services. The Community Cares model of service delivery supports the provision of timely, comprehensive, high-quality chronic care management services that operate under a whole person approach to care. The whole-person approach to care addresses all of the clinical and non-clinical care needs of the individual.

Duties and Responsibilities

1. Referral tracking.
2. Training and technical assistance.
3. Data management (including processing enrollment/discharge/transfer forms).
4. Scheduling for primary care community care team and enrollees.
5. Chart audits for compliance.
6. Reminding enrollees regarding keeping appointments, filling prescriptions, etc.
7. Requesting and sending medical records for care coordination.
