Catholic Health Services Outpatient Clinical RN in West Islip, New York

Overview Provides direct patient care, in a medical/physician office setting, using the nursing process in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization. Work Schedule: Day Shift – Mon/Fri 9-5pm (Flex) Job Details Assists/Facilitates the intake of new patients Plans, implements, and evaluates patient care plans based on patient assessment to optimize outcomes and maximize available resources. Monitors, records, and communicates patient condition as appropriate. Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs. Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances. Facilitates clinical operations and patient care: Maintains and is responsible for efficient and appropriate patient flow including but not limited to rooming in and out of exam rooms. Facilitates consultations, pre-testing, diagnostic tests or procedures, referrals and future appointments for patients. Assists Breast Surgeon in various Outpatient Patient Procedures Triages patient phone calls as needed and documents relevant information in the patient's chart. As necessary, facilitates communication between patient/family and interdisciplinary team and coordinates services related to patient care. Communicates imaging and pathology results on behalf of provider and document in EMR Team player who contributes to building the team by helping others succeed through role modeling and mentoring. Considers maintenance of a safe environment, patient condition, complexity of the intervention and predictability of the outcome. Shares knowledge and skills with peers and colleagues and cultivates a work environment that promotes positive employees attitudes, effective communications, trust, and collaboration. Collaborates with an interdisciplinary team to evaluate clinical care or health services. Promotes patient safety initiatives. Participates in department specific quality initiatives. Assures appropriate order sets are used. Analyzes the health status of the individuals and families comparing the data with the norm, when possible, to determine patient care needs. Assesses patient reports/records change of condition and notifies physician and/or supervisor and intervenes appropriately and documents accordingly. Accepts assignments in areas other than primary unit, as well providing coverage for other members of the Breast Surgical Team. Seeks both formal and independent learning activities using current research findings and other evidence to maintain and develop clinical and professional skills and knowledge. Participates in systematic peer review and obtains informal feedback regarding one’s own practice from patients, peers, professional colleagues and others. Identifies areas of strength, as well as, areas in which professional development would be beneficial. Participates in department specific patient satisfaction initiatives. Assesses for and attempts to resolve patient/family concerns. Utilizes effective time management skills to complete assigned duties and responsibilities within established work shift. Partners with Administrative Manager and Physician team to assist in clinical projects as needed to meet the goals of the Breast Surgery Service Line. Requirements: New York State license as a Registered Professional Nurse required Baccalaureate Degree in Nursing Knowledge of departmental policies and procedures, regulatory codes, personnel and administrative policies required Minimum of 2 years' experience as a RN required Minimum 3 years Clinical Nursing experience in acute care setting, knowledgeable regarding the care of Breast cancer patients preferred Oncology Certification in Breast Care and/or Oncology Nurse Navigator Certification, preferred. Salary Range USD $42.00 - USD $65.00 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies, and experience and position location. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
