The Behavioral Health Social Worker is a master's level behavioral health professional who conducts clinical assessments and provides brief interventions, care management, navigation and proactive follow up for patients with comorbid medical, mental health and/or substance use disorders. The Behavioral Health Social Worker leverages their behavioral health expertise in providing care to patients and families, improving transitions of care and assuring that appropriate follow up care is established to assure the best possible clinical outcomes for at risk patients. The Behavioral Health Social Worker is also competent in non-behavioral health related resources and services (medical home care, office of aging, children and youth, domestic violence, etc) to facilitate discharge planning in the Emergency Department and other hospital-based settings.

· Conducts biopsychosocial assessments with patients identified/consulted on by providers, nurses, or request of patient, family member, or designated caregiver, etc.

· Administers suicide risk assessments with patients identified/consulted as at risk for suicide or is seeking assistance with behavioral health related symptoms/concerns.

· Determine the least restrictive level of care based on information available from the patient or collateral contacts, and in collaboration with patient’s treatment team.

· Coordinate discharge plans and plans for transfer as appropriate based on level of care recommendations, resource needs, etc. Tasks include insurance authorizations, coordinating appointments, making referrals, coordinating transport arrangements, arranging outpatient behavioral health, medical resources (homecare, hospice, etc), or social needs related care (Children & Youth, Office of Aging, Domestic Violence, etc.)

· Completes or assists others in completing necessary psychiatric commitment paperwork based on patient needs, including coordinating and testifying in mental health court hearings as needed.

· Assists colleagues in responding to and managing clinical crises; including but not limited to serving as part of the Behavioral Response Team (BRT).

· Serves as a behavioral health advocate and liaison interfacing with medical providers and health care staff across the continuum of care, in the emergency department, hospital settings, primary and specialty care. Coordinates care across the continuum (inpatient/outpatient/community) to assure appropriate utilization of clinical and community resources.

Participate in hospital-based, health system-based, or community-based workgroups and committees.

Master's degree Social Work required

PA License of Social Work (LSW) preferred
