What You Will Do: The Telemonitoring RN monitors patients\' vital signs/trends and provides appropriate triage and follow-up with physicians and other care team members. There are two main goals that the Telemonitoring RN assists with: daily remote monitoring to identify true changes in condition and provide appropriate early intervention while successfully educating the patient. This education is done in a comprehensive teaching method to ensure that the patient is able to independently manage their health after discharge from the Telemonitoring program. As an integral part of the Telemonitoring Program, this position has a high level of responsibility and therefore requires an individual who is independent, communicates well with other staff members and physicians, and is a respected and trusted member of the care team.
