Description

San Mateo Medical Center (SMMC) , part of San Mateo County Health, is seeking a qualified and experienced Diabetes Nurse Educator for its Innovative Care Clinic's (ICC), Diabetes Self-Management Education. The Innovative Care Clinic (ICC) is a fully functional adult care clinic providing an interdisciplinary and integrated model of care emphasizing collaboration across professional roles. The Diabetes Nurse Educator will also serve the Ron Robinson Senior Care Center, Coastside, and Mental Health Primary Care clinics.

The Diabetes Nurse Educator functions as a patient educator collaborating with patients with a diagnosis of diabetes to educate them on areas such as weight management, food intake, medications and the proper usage of a glucometer. The goal is to assist patients with high A1C to lower their numbers to be within normal range. The Diabetes Nurse Educator will follow the American Diabetes Association guidelines for Diabetes Self-Management Education (DSME). The educator is responsible for submitting regular data to the ADA to retain SMMC's certification. The educator will lead a team of diabetes educators across the SMMC system by conducting regular meetings to discuss any updates on DSME.

The ideal candidate has at least one year of experience in ambulatory care setting addressing chronic disease management; is skilled in patient diabetes self-management education, triage, and assessment; has worked with the underserved population and on interdisciplinary teams. The ideal candidate is also a critical thinker, fast learner and thrives in a dynamic environment. Computer literacy, prior experience with electronic medical records (E-Clinical Works and/or EPIC preferred). Experience in data tracking/management is highly desirable. Bilingual in Spanish and English is preferred.

Additionally desired Skills/Abilities/Knowledge:

  • Certified Diabetic Educator Certification (preferred but not required)

  • Proficiency in using Excel in order to track key data for ongoing certification with the American Diabetes Association.

  • An individual assessment and education plan developed collaboratively by the individual and educator(s) to direct the selection of appropriate educational interventions and self-management support strategies.

  • Educational interventions directed toward helping the individual achieve self-management goals.

  • Able to conduct one to one and group educational classes.

  • Periodic evaluations to determine attainment of educational objectives.

  • Documentation of the patient interaction and education plan, as well as any need for follow-up visits.

  • Ability to communicate effectively to patients in regard to nutrition, diet, and exercise to improve their overall health and quality of life.

Examples Of Duties

Duties may include, but are not limited to, the following:

  • Identify problem situations or patient care issues; analyze, interpret, advise and consult with primary care providers to provide high quality, contemporary care.

  • Interpret and modify the system of patient care delivered by evaluating and reviewing standard practices and initiating new procedures.

  • Provide consultation and guidance to all levels of the nursing staff pertinent to patient care problems within the field of specialty.

  • Advise the nursing staff in the establishment and maintenance of records of treatments given and the related progress of each patient for the purpose of analyzing patterns, synthesizing actions and evaluating patient care plans.

  • Resolve unusual or unique patient care problems or situations, which require analytical, interpretative and constructive thinking.

  • Formulate and develop plans, procedures and program content designed to meet specifically identified training/educational needs of patients.

  • Formulate, and/or collaborate with nursing leadership to evaluate the DSME Program for any needed updates.

  • Present data and/or other DSME related updates in professional meetings.

  • Evaluate patient-facing literature and resources to affirm they are meeting language needs and cultural competence.

  • Serve as role model and resource person for staff and members of the interdisciplinary team.

  • Perform related duties as assigned.

Qualifications

Licensure/Certification:

  • A California license as a Registered Nurse.

  • Possession and maintenance of BLS certification.

Education and Experience:

  • Associate Degree in Nursing with two years clinical experience including one year in a clinical specialty area, or

  • Baccalaureate Degree in Nursing or a health-related field with one year of clinical experience including six months in a clinical specialty area, or

  • Master's degree in Nursing.

  • Must have knowledge of and verifiable experience in adult education curriculum development, program evaluation, and evidence-based and best practice implementation.

Knowledge of:

  • Advanced principles and practices in specialty areas.

  • Nurse Practice Act.

  • Current nursing and related medical theory and skills necessary to administer nursing care within assigned clinical setting.

  • Safety and infection control practices.

  • Clinical systems, supplies and equipment.

  • Modern principles, techniques, and procedures of nursing.

  • Medical terminology.

  • Use and maintenance of medical instruments and equipment.

  • Use and effects of medicines.

  • Public health principles

  • Health promotion and disease prevention.

  • Medical research principles and practices.

  • Principles of training and education.

  • Lean principles and methodologies.

  • Data abstraction and utilization.

  • Basic statistics for reporting purposes.

  • Report writing.

  • Regulatory and accreditation requirements for assigned area.

  • Evaluate the work of others.

  • Develop and present training and continuing education programs.

  • Design, evaluate, construct and modify research models; carry out research and present findings.

  • Perform the full range of nursing duties and responsibilities.

  • Prepare clear and concise medical record documentation.

  • Write articles for publication on areas of specialty.

  • Identify problems or discrepancies and make suggestions for corrective actions.

  • Teach and orient staff.

  • Establish and maintain effective working relationship with patients and staff members.

  • Communicate effectively.
