    Position Summary:

    LPN will be paired with a RN or UNL to assist with the patient load. LPN tasks may include oral meds, IV piggyback meds, trach care, dressing changes, wound care, hourly rounding, reinforcing patient education and completion of discharges. LPNs will document assessments and care plans after the RN has completed the initial assessment and initiated the care plan.

    Qualifications:

    EDUCATIONAL REQUIREMENTS Diploma (Not HS) in Nursing CERTIFICATION & LICENSURE REQUIREMENTS Licensed Practical Nurse (LPN) Illinois upon hire and Basic Life Support (BLS) within 30 days. EXPERIENCE REQUIREMENTS None Specified

    Essential Functions:

  • Administers medications, including IV piggybacks and fluids but excluding IV Push medications

  • Documents assessments on assigned patients, given that an RN has charted the initial assessment

  • Charts on care plans, which the RN has opened; will have a RN open any new care plans

  • Communicates with providers to receive orders and coordinate care for patient

  • Completes dressing changes and wound care

  • Completes trach care

  • Inserts NG tubes and maintains feedings through NG tube

  • Cares for and changes ostomies

  • Reinforces education and discharge instructions
