St. Luke's Health System Ambulatory Care Pharmacist - Pharmacotherapy Clinic in Twin Falls, Idaho

St. Luke's is seeking an Ambulatory Care Pharmacist to join our team. This position will be a part of our Pharmacotherapy Pharmacist team in Twin Falls. This role is a vital part of the health care team within our primary care and cardiology service line!

What you can expect

  • Work in a pharmacist-led outpatient pharmacotherapy clinic, which accepts referrals for anticoagulation management AND primary care chronic disease state management

  • Work under a broad collaborative practice agreement to provide medication management

  • Manage a fast-paced clinic schedule

  • Provide support to providers and patients through answering drug information questions, trouble-shooting pharmacy problems and educating on medications

  • Coordinate with the multidisciplinary team of physicians, advanced practice practitioners, behavioral health specialists and care managers

  • Collaborate with clinical pharmacy technicians

  • Opportunities for precepting pharmacy students and residents

Qualifications

  • Degree from an accredited pharmacy program

  • Valid Idaho Pharmacist License

Preferred Qualifications

  • Previous experience with the clinical management of anticoagulation, hypertension, diabetes and other chronic disease states

  • Completion of PGY2 Ambulatory Care ORcompletion of a PGY1 pharmacy residency program with ambulatory care-focus ORequivalent ambulatory care experience

  • Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), Geriatric Pharmacy (BCGP) or Cardiology (BCCP)
