Behavioral Health Connections (BHC) in Phoenix is seeking a dynamic and talented Referral Management Clinician that serves to assess and coordinate the care of patients referred for Mental Health Services. They review referral information, determine the appropriateness of services, and facilitate the connection between patients and the most appropriate facilities or treatment programs. This role is crucial in ensuring patients receive timely and effective care tailored to their specific behavioral health needs.

Job Duties/Responsibilities:

  • Perform all aspects of the hospital’s assessment and referral process.

  • Service coordination by matching patients with appropriate mental health services and providers within UHS, considering factors such as specialty, location, and systems complexities. and insurance coverage.

  • Responsible for providing and assisting patients with resource navigation, assisting patients in understanding their treatment options and guiding them through the healthcare

    Qualifications

    EDUCATION

    Master’s degree preferred in Social Work, Psychology, Counseling, Behavioral Health or related field.

    LICENSURE/CERTIFICATION

    Preferred Current license in the State of Arizona as a Licensed Professional Counselor (“LPC”), Licensed Master Social Worker (“LMSW”), Licensed Clinical Social Worker (“LCSW”). Or Licensed RN or LPN

    Current CPR, Vaccines, if required by a customer hospital. (“Customer hospital” means the hospital to which BHC is contracted to provide mobile assessment services.)

    Must have a valid driver’s license.

    EXPERIENCE

    Preferred 1-year work experience working with Assessment and Referral department in a Behavioral Health Setting.

    Completion of appropriate internship, including the provision of individual, group or family therapy/counseling.

    Experience providing individual, group and family therapy/counseling.

    Mobile assessment experience preferred
