SUNY Upstate Medical University RN Navigator - Hepatobiliary Program in Syracuse, New York

Job Summary:

The RN Navigator for the Hepatobiliary Program is a Registered Nurse whose clinical nursing experience guides patients and their families to informed decision making. Assumes responsibilities and accountability for planning, guiding and evaluating the delivery of patient care. Monitors patient's progress throughout treatment. Intervenes as needed to ensure compliance and optimum care. Collaborates within a multidisciplinary team to facilitate timely, appropriate screening, diagnosis, therapeutic intervention, treatment and supportive care across the health care continuum. The RN Navigator assumes responsibility for the coordination of all resources necessary to provide care. Utilizes advanced expertise to identify, implement and improve processes in order to provide an integrative model of care to the patients, while assuring the highest level of patient satisfaction. The RN Navigator provides patient education for each patient regarding their disease process and treatment throughout the health care continuum. Demonstrates and promotes professional development. Maintains a professional, safe and therapeutic environment for rendering care.

Minimum Qualifications:

NYS licensed/registered (or eligible) RN required. Bachelor's degree in Nursing or healthcare-related field, three (3) years related RN experience required (preferably in specialty) OR an Associates degree in Nursing and five (5) years related RN experience (preferably in specialty) required. Excellent written/oral communication and interpersonal skills required. CPR-BLS certification is required by the time of appointment.

Preferred Qualifications:

Three (3) years as a Certified Nurse /Patient Navigator in a specialty area preferred. Acute care RN experience preferred.
