POSITION SUMMARY: Responsible for providing intensive casework, assessment, intervention and referral services to patients

and/or their families. Emphasis is placed on providing for patient’s social needs, exercising initiative and independent judgement

based upon professional knowledge, experience and hospital policy. Liaison to other services along the continuum of care and

community support services.

EDUCATION, TRAINING, EXPERIENCE, CERTIFICATION AND LICENSURE:

Masters Degree in Social Work from an accredited college or university, and passing exam offered by ASWB and licensure as LMSW

or permit to practice as LMSW through State of New York. Must receive license within 12 months following issuance of limited

permit, if new hire does not have license at beginning of employment.

Participates in orientation and continuing education and updates and maintains knowledge and skills related to specific areas of

expertise.

SPECIAL EQUIPMENT, SKILLS OR OTHER REQUIREMENTS:

Crisis intervention skills. Ability to communicate effectively with patient, family, medical team and other professions within hospital

and in the community.

WORK ENVIRONMENT AND HAZARDS:

Office and clinical setting. Exposure Class II. No exposure to blood, body fluids, excretions or secretions.

PHYSICAL DEMANDS:

Light work: May work long and irregular hours in an emotionally stressful environment.

WORK CONTACT GROUP:

All services, employees, medical staff, patients, visitors, vendors, various regulatory and professional agencies.

Pay Range: $24.60-$41.40

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location
