PM shift pays \$17.74 / hour plus a \$2.00 / hr shift premium so you make \$19.74 / hour! PM shift hours range between 2:30 pm - 10:45 p.m. 3rd shift pays \$17.74 / hour plus a \$.75 / hr shift premium so you make \$18.49 / hour! Summary of Position: The CNA, under the direction and supervision of the professional nurse, performs direct resident care, including the provision of personal care, assistance with activities of daily living, and the documentation and the reporting of those activities. The CNA upholds the philosophy of Portage County Health Care Center to provide rehabilitative service necessary for each resident to achieve a maximum level of functioning, the preservation of self worth and dignity.
