Responsibilities Job Description

As a member of the primary care team at M Health Fairview, the Behavioral Health Clinician assists in the team’s management of the overall health care needs of the enrolled patient population! The Behavioral Health Clinician’s daily tasks include assessment, consultation and direct care related to the prevention, identification, treatment and management of psychosocial/behavioral concerns which impact the health of the defined patient population.

Conducts assessment and evaluation of mental health conditions by means of diagnostic and screening tools, clinical interviews and consultation with the medical team and documents assessments in medical record confers diagnosis as appropriate.

Collaborates with patient, family, and other colleagues to develop a treatment plan of care! Develops an individualized plan based on age, social and cultural considerations, condition, and other relevant needs. Maintains required, and timely communication and correspondence with treatment authorization and payer sources in a manner which promotes efficient delivery of clinical services.

Contributes to the development and implementation of treatment plan in collaboration with the patient and care team.

Provides consultation to the team sought at the prevention, identification, treatment and management of behavioral health and psychosocial issues and conditions.

Functions as an effective team member as evidenced by working in a collaborative approach with primary care providers.

Demonstrates knowledge of psychotropic medications.

Understanding of integrative health care in terms of the interactive and interdependent expressions of the physical/biological, emotional and psychological dimensions of health.

Employs a primary care approach in addressing the behavioral/psychosocial aspects of routine and complex medical conditions.

Provides behavioral health interventions for identified mental health and substance use conditions.

Delivery of population based health care by providing individual, group and community-based services. Maintains concise, timely and accurate documentation in the patient’s medical record.

Connects patients with community resources as needed to meet living and support needs.

Optimally refers and leads collaboration and communication across the continuum of care.

Qualifications

Job Qualifications :

Masters in social work or psychology with emphasis on clinical practice. MN state license, eligible to be licensed in the State of Minnesota and eligibility to become a credentialed provider with insurance entities. Experience in providing diagnostic assessments and direct evidence-based clinical services to children and adults. Experience with Electronic Health Record systems preferred.

Required:

  • Masters or Doctoral Degree in Social Work or Psychology

  • Licensed Social Worker (LICSW) or Doctoral Level Licensed Psychologist (LP), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC)

  • 2 years post licensure

Preferred:

  • 5 years post LICSW / LP licensure

  • Previous experience completing diagnostic evaluations, and providing individual, couples, family counseling for children and adults

  • Healthcare experience

  • Knowledge of health conditions and common co-morbidities

  • Knowledge of the continuum of behavioral health care services (i.e. within surrounding communities)

  • Experience with Epic
