Joining forces with Elara Caring as a Hospice RN Case Manager allows you to extend exceptional care across the entire care continuum. As an RN Case Manager in our Hospice service line, you'll make a profound impact by delivering compassionate care to patients in the comfort of their homes.

Schedule: Full Time, Monday through Friday

Responsibilities:

  • Manage and coordinate patient care for hospice patients.

  • Conduct thorough patient assessments and develop personalized care plans.

  • Provide nursing care adhering to hospice regulations and standards.

  • Collaborate with interdisciplinary team members to ensure comprehensive patient care.

  • Utilize medical terminology and physiology knowledge to deliver superior patient service.

  • Facilitate smooth transitions for patients through effective discharge planning.

  • Uphold HIPAA regulations and maintain patient confidentiality.

  • Utilize critical care and ICU experience as needed.

  • Address behavioral health needs with compassion and expertise.

  • Demonstrate proficiency in home health care and hospice services.

  • Maintain RN license and stay abreast of nursing best practices.

What is Required?

  • Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing

  • Current State Licensee as a Registered Nurse RN

  • 1 year of experience in a clinical care setting

  • Experience in hospice or home health environment is preferred

  • Nursing skills as defined by accepted nursing standards

  • Ability to handle multiple priorities, documentation requirements, and deadlines

  • Reliable transportation to perform job tasks
