Essential Functions
Maintain current knowledge of trends, expectations and changes in clinical practice, regulation, and policy as it relates to delivery of care, quality, and reimbursement
Provide consultation, guidance, education, and completes on-site assessments related to quality, safety, regulatory standards, reimbursement processes, and staffing standards.
Provide professional, quality-driven services, including but not limited to mock surveys, education, research, analysis of healthcare systems and processes, creative problem-solving and recommendations to Presbyterian Homes staff
Monitor and evaluate clinical programs and systems and assist with corrections and training
Regularly provides feedback and corrective action to Vice President of Clinical Services on overall clinical, quality, safety, and reimbursement process
Provide guidance by collaborating with community leadership to reduce risk and improve quality, safety, and satisfaction
Provide education to ensure employees are informed of changes in regulations, best practices, policies, and practices
Assists with the development and revision of clinical policies and procedures to ensure compliance with governmental regulations and current standards of practice
Implementing systems, monitoring, and developing improvement plans in order to promote positive resident outcomes
Covers in the absence of an MDS Coordinator or another clinical leader in the communities as directed.
Acts as a support system to the facilities as needed throughout the state survey process, including assistance with survey preparation, writing a corrective action plan, and monitoring for compliance
Consult and provide technical or interim management assistance to communities as requested
Skills/Qualifications
A bachelor’s degree required.
Current RN license by the State of Illinois required
Resident Assessment Coordinator Certified (RAC-CT or RAC-CTA) required
5-10 years’ recent clinical and leadership experience in Long-Term Care setting, preferably in the State of Illinois
Director of Nursing or Licensed Nursing Home Administrator experience preferred
Proven experience with quality and performance improvement (QAPI) concepts and processes
