  Job Summary

• The Bereavement Coordinator is responsible to plan, implement, and maintain a bereavement program to meet the needs of families/caregivers from Avera@Home for up to one year following the death of the individual hospice patient.

  Education and/or Experience

• Bachelor’s degree in Social Work, Counseling, Theology, Pastoral Care or a related field is required. Master’s degree is preferred.

• Experience with families, caregivers and patients through death/dying issues is preferred.

• Experience with clinical grief counseling is preferred.

• Experience in facilitating grief support groups is preferred.

  Licensure/Certification/Registration

• Certification in Thanatology is preferred.

• This position requires a valid driver’s license and that the employee is insurable by the Avera Health automobile liability insurance carrier.

  Work Schedule

• Monday-Friday, 8:00am-4:30pm with no call involvement; 80hrs/2wks

• Call time involvement: weekly for home admissions

  Pay

  The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

• $20.50 – $31.00 per hour
