Description:

Under limited supervision, provides medical support to assigned patients by utilizing the nursing process and meeting the established standards within the clinic. Is accountable for assigned patients, demonstrates understanding of nursing and its issues and is an advocate for patient/families and nursing. What you will do:

LPN

  • Demonstrates the knowledge and skills necessary to provide service/care appropriate to the age of the patients served, in accordance with MercyOne standards.

  • Utilizing the nursing process assists the registered nurse/provider in providing patient care.

  • Demonstrates ability to identify physical, psychological, social and spiritual needs of the patient and appropriately reports to the registered nurse/provider.

  • Promotes and maintains a safe and calm environment for complex patients by ensuring appropriate supplies, equipment, and necessary services.

  • Identifies actual and potential risk to patients and/or families and initiates appropriate action to minimize risk.

  • Administers medications and treatments/procedures as directed, and as appropriate for the patient’s medical care, in collaboration with the multi-disciplinary team.

  • Utilizes internal and community resources in planning care and initiating referrals.

  • Initiates appropriate patient/family education following assessment of learning and discharge needs.

  • Documents nursing intervention and patient/family response accurately, legibly, and in compliance with clinic policies.

  • Appropriately delegates tasks/procedures to other team members who are prepared/qualified to perform.

  • Demonstrates the skill level requirement for a Licensed Practical Nurse as evidenced by competent performance and completion of the skills checklist.

  • Contributes to the learning experience of students and staff.

  • Uses effective communication principles to coordinate care with other health care providers.

  • Actively participates in clinic meetings, committees, conferences, and in-services.

  • Recognizes and communicates issues/occurrences utilizing established lines of authority and assists in identifying ways to resolve variances or unacceptable outcomes.

  • Is knowledgeable of and actively participates in improving patient safety and reducing risk to patients. Complies with Safety and Infection Control & Procedure.

  • Knows, understands, incorporates, and demonstrates the MercyOne and Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions.

  • Maintains a working knowledge of applicable Federal, State, and local laws and regulations, MercyOne and Trinity Health’s Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.

RN

  • Demonstrates the knowledge and skills necessary to provide service/care appropriate to the age of the patients served, in accordance with Mercy standards.

  • Obtains vital signs, blood pressures, heights, and weights and records information appropriately, accurately and in a timely manner.

  • Demonstrates competency in performing phlebotomy, use of appropriate tubes for blood specimen collection, labeling, accurate identification, etc.

  • Assists with patient treatments as delegated, i.e., specimen collection, simple dressing changes.

  • Processes simple laboratory specimens. Maintains CLIA/COLA evaluation and documentation of competency.

  • Performs screen and follow up of patient test results.

  • Assists with patient exam as directed by the physician.

  • Obtains EKG tracing as directed and ordered by physician.

  • Identifies and communicates significant changes in patient’s status (potential or actual adverse patient occurrences) to the physician.

  • Communicates and collaborates effectively with multi-disciplinary team members as well as patients, families and significant others.

  • Transports patient to and from other departments as delegated.

  • Initiates appropriate measures to ensure safety/well-being of patient, family, visitors and staff.

  • Maintains cleanliness and organization of patient environment, utilizing the established room cleaning procedures and appropriate supplies.

  • Is knowledgeable of and completes clinic specific responsibilities/assignments.

  • Performs clerical duties as assigned, i.e., scheduling of appointments, verification of patient demographics, assisting with the maintenance of medical records.

  • Uses effective communication principles to coordinate care with other health care providers.

  • Actively participates in clinic meetings, committees, conferences, and in-services.

  • Recognizes and communicates issues/occurrences utilizing established lines of authority, and assists in identifying ways to resolve variances or unacceptable outcomes.

  • Knows, understands, incorporates, and demonstrates the Mercy and Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions.

  • Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Mercy and Trinity Health’s Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.

  • Maintains confidentiality of information pertaining to clients, physicians, employees, and Mercy business.

  • Demonstrates general working knowledge of computers and department specific software (i.e. Healthstream, internet, etc.)

  • Performs other duties and responsibilities as assigned.

What we are looking for:

LPN

  • Must be a Licensed Practical Nurse maintaining current state licensure.

  • A minimum of one to two years nursing experience preferred.

  • Basic Life Support Certification (BCLS) is required unless doctor’s release is provided.

  • Is able to communicate effectively with all members of the health care team.

  • Must have a general working knowledge of computers and department specific software (i.e. Healthstream, internet, etc.)

  • Must be comfortable operating in a collaborative, shared leadership environment.

  • Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of MercyOne and Trinity Health.

RN

  • Must be a registered nurse maintaining current state licensure.

  • One to two years nursing experience preferred.

  • Basic Life Support Certification (BCLS) is required unless doctor’s release is provided.

  • Is able to communicate effectively with all members of the health care team.

  • Maintains a high level of professionalism, which positively promotes the image of MercyOne.

  • Must have general working knowledge of computers and department specific software (i.e. word, excel, bed tracking, Internet, med aid, etc.)

  • May be requested to travel within the Siouxland region. When assigned to travel, a valid drivers license and proof of insurance will be submitted to manager prior to travel.

  • Must be comfortable operating in a collaborative, shared leadership environment.
