This is a Monday through Friday opportunity to work with a dynamic team of clinical professionals at a well respected healthcare organization at our Cancer Radiology Center. Will work for health and wellness, survivorship and post- hospital patient navigation and case management.

Location: Holy Cross Health has two hospitals and four healthcare centers all a short driving distance from Washington DC and Baltimore, MD

The Oncology Social Worker provides assessment, counseling, referral and psychosocial support to patients and their families who need assistance in handling emotional or financial distress or coping needs associated with cancer and cancer treatment. Facilitates the development of support services and programs designed to achieve optimal outcomes for cancer patients and their families. Provides expertise to the Cancer Committee, community, and physicians on psychosocial needs and resources available for cancer patients, families and caregivers. Establishes trust with a diverse population of cancer patients to improve their navigation of support and resources.

Supports the Mission of Trinity Health and Holy Cross Hospital.

Supports the Strategic and Operational Plans for the Holy Cross Cancer Institute.

Description:

  • Reporting to the Manager of Social Work

  • Social Worker will provide assessment, counseling, referral and discharge planning services to all age groups of patients treated at Holy Cross; provides supportive services to patients and their families.

  • Manage Oncology Referrals and outpatient oncology patients. Assists in providing guidance to patients to overcome barriers related to Oncology Care.

Responsibilities:

  • Social Workers are focused on discharge and initial inpatient admission and appropriate community-based services.

  • Reviewed clinical assessments and determining appropriate levels of care, determining if additional clinical services are needed for infants to elderly patient population.

  • Effectively handled member crisis calls by identifying risk, ensuring safety, and coordinating timely access to care.

What you will need:

  • State of Maryland LSWA, LGSW, LCSW or LCSW-C licensure (or license eligible).

  • Bachelors or Masters Degree in Social Work.

  • One year of social work experience in an acute-care setting preferred. Demonstrated ability to work with all age groups of patients, with an emphasis on adult and geriatric patients. Able to communicate effectively.
