Better Life Homecare Agency Quality Improvement Manager in Sharon Hill, Pennsylvania

TITLE OF IMMEDIATE SUPERVISOR: AGENCY MANAGER

RISK OF EXPOSURE TO BLOODBORNE PATHOGENS - LIMITED

Agency Management:

  • Assists with developing and implementing the Agency's Quality Improvement Program.
  • Assists in the implementation of approved recommendations and assessing the Agency's success in correcting problems.
  • Reviews all reports of complaints, errors, incidents, and grievances to insure that they are fully documented and properly handled in the time-frame and manner specified by the Agency's policy.
  • Implementing Governing Body directives and ensuring that appropriate service policies are developed and implemented.
  • Establishing and maintaining effective channels of communication; maintaining liaison with the Governing Body.
  • Ensuring staff development including orientation, in-service education, continuing education and evaluation of Steff are current and include Quality Improvement initiatives.
  • Directing and monitoring organizational Performance Improvement activities.
  • Coordinates a program for risk management which encompasses written guidelines and provides for supervision of workers on cases with risk factors, such as: contagious disease, infections, AIDS, hepatitis, TB and other situations in which the consumer or worker is at risk.
  • Makes home visits regularly to observe care being rendered to assess the condition and use of equipment and supplies, and to receive reports of the consumer/family satisfaction with care/services.
  • Assures the confidentiality of consumer records.
  • Promotes and maintains an Agency environment that is in compliance with federal, state and local regulatory agencies.
  • Consults with staff when necessary to ensure and maintain safe, efficient and quality consumer care/services.

Reviews at Least Monthly:

  • The timeliness cf the admission process.
  • The appropriateness of the amount, frequency, duration, and level of care authorized.
  • The completion of all assessments, assignments, care/service plans, and other required documentation.
  • The fulfillment of services: type and frequency.
  • Sampies of discharged cases to verify that a summary has been completed and that appropriate disposition of the case has been made and is documented.

Reviews at Least Annually:

  • Preparing, distributing, tabulating, and evaluating of the results of questionnaires directed to consumers, families, employees/contractors and referral sources to determine their satisfaction with the Agency's services.
  • At least annually, reviews all job descriptions to insure consistency with recruitment and hiring practices, and with state and national requirements.

JOB CONDITIONS

  • Position is stressful in terms of meeting deadlines.
  • It requires minimal lifting of office records and printouts.
  • It is primarily a desk job which essentially involves sitting, standing stooping and walking, as well as an inordinate amount of telephone communication.
  • Travel is required, by car or airplane to seminars, conferences or meetings.
  • The ability to communicate well, both verbally and in writing in English is required

EQUIPMENT OPERATION

  • Utilization of CRT, calculator, multi-line telephone, copy machine, basic equipment.

COMPANY INFORMATION

  • Has access to all consumer records, personnel records and consumer financial accounts which may be discussed with the Executive Director, Director of Finance, Employee Relations and Qua!ity Assurance department staff, as well as outside government agencies.

QUALIFICATIONS

1 . A licensed Registered Nurse or Licensed Practical Nurse.

  1. Must have a minimum of 2 years of home care experience.

  2. Must have experience in developing, implementing, and evaluating continuous quality improvement programs and activities.

  3. Must have the ability to demonstrate self-confidence and a positive attitude and maintain enthusiasm and commitment with respect to goal achiev ment.

  4. Must have a criminal check and other checks as required by PA regulation.
