Ellis Medicine Clinical Resources Coord - Primary Care in Schenectady, New York

The Registered Professional Nurse (or similar) functioning in a Clinical Resource Coordinator position cares for patients by diagnosing and treating actual or potential health problems in the ambulatory setting without discrimination to age or gender through implementation of the nursing process.

Coordinates care of other disciplines, delegates tasks to other nursing staff members, supervises care and evaluates results. Maintains competence in clinical practice and assists in the development of nursing practice standards through participation in professional committees and organizations. In addition, this individual participates in the evaluation of members of the clinical team, the receiving and investigation of complaints of a clinical nature, and quality improvement activities. He/She serves as a resource to the on-site clinical team and is a conduit with other sites/departments related to clinical activities. The Clinical Resource Coordinator reports to the Practice Manager and works with the Lead Clerical Coordinator, Practice Manager and Director to make decisions that impact the practice.

Requirements:

Registered Nurse with current New York State licensure. Minimum three (3) - five (5) years' nursing experience in an office/clinic setting preferred. The Registered Nurse is expected to complete hospital orientation upon hire, annual mandatory assignments and maintain current BLS certification. Continuing education and professional organization involvement is encouraged.

Responsibilities:

  • The Clinical Resource Coordinator will complete office patient’s initial assessment using subjective and objective data sources.

  • Deliver patient care at the direction of attending MD/NP.

  • Will develop a NCP based on patient assessment. Particular attention is given to developmental stages of the patient/family

  • Effectively communicate the patient's plan of care to all members of the health care team.

  • Ensure operational flow is maintained in the office setting.

  • Organize and assign tasks to support staff, appropriate to their education and skill level, to maintain operational flow and compliance with office policies and procedures

  • Provide patient education focusing on wellness health maintenance and restoration.

  • Assume responsibility for education and self development.

  • Maintains competency in nursing practice, and keeps up with new knowledge and advances in the profession, and completes the required mandatory programs.

  • Functions within the standard of practice, policy, procedures and guidelines of the nursing department.

  • Participates in effort to continuously improve unit operations.

  • Adheres to the policies and procedures as outlined in the Exposure Control Manual regarding: universal precautions, blood-borne pathogen; disposal of needles; syringes and sharps; cleanup procedures for blood and body fluid spills; storage, handling and disposal of medical waste. Appropriate personal protective equipment is utilized when performing a task or responsibility associated with this position which represents a potential occupational risk of exposure to blood-borne pathogens. Appropriate PPE’s shall include but not be limited to gloves, gown, goggles, laboratory coat, face shields or masks.

    Salary Range: $29.44-$44.16/hour Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
