Rotating shifts and areas with a primary focus in oncology and CAR-T program patients. Preparation and dispensing of drugs in all inpatient settings; maintenance and review of patient medication profiles; reviewing and verifying physician medication orders and patient medical record. Interacting with healthcare professionals and providers to help promote safe medication use, prevent inappropriate or misuse of medications; provide clinical pharmacy services (e.g., pharmacokinetic dosing, therapeutic interchange, IV/PO conversions, TPN per pharmacy, clinical protocols per pharmacy and pharmacist interventions.

When assigned to decentralized duties in patient care and Oncology units, the pharmacist will be physically located in the patient care areas and primarily responsible for verification of new admission medication orders, medication history taking and reconciliation of high risk patients upon admission, and rounding with MD/RNs beginning on admission to provide effective medication therapy during the patient's stay and early planning for discharge.

Hourly Salary Range: $83.37 - $97.65 (Union represented - step based - based on years of experience)

Bargaining Unit: HX - UPTE

Schedule: Varied shifts, depending on department needs including weekends and holidaysQualifications

We're seeking a dedicated clinical professional with:

  • Pharm D. degree and current CA Pharmacy license.

  • Exceptional communication, interpersonal, organizational and follow-through skills.

  • Knowledge of and ability to perform age specific and disease specific pharmacokinetic calculations.

  • Ability to compound intravenous admixtures and other sterile products.

  • Completion of PGY1 Pharmacy residency or equivalent 3-5 years of experience in inpatient hospital pharmacy setting and/or in decentralized patient care pharmacy setting.

  • Clinical background/residency in oncology and/or Board Certification in Oncology preferred.
