UCLA Health Homeless Healthcare Collaborative Care Manager in Santa Monica, California

Description

The Homeless Healthcare Collaborative care manager functions as a part of the street medicine mobile clinic program. The care manager will report to the HHC Care Management Program Manager with a primary duty to provide enhanced care management (ECM) by assisting unhoused clients with navigating systems of care and accessing resources that address social determinants of health.

The care manager will outreach and collaborate with a multidisciplinary street medicine care team to recruit, enroll, and provide case management services to qualifying individuals for medical and social services through enhanced care management. They will work with clinicians and community health workers to determine the acuity of need for patients and provide patient tailored enhanced case management for these individuals that addresses both clinical health needs and the social determinants of health.

Case management will include development of a care plan and assisting clients with linkages to medical, psychiatric, social, educational, housing, food, and other services as deemed necessary by the care team and the patient. They will document and track the patient's progress and goals. Care managers will work at HHC's headquarters and will travel to community sites as needed and work alongside the program's clinical teams, patients, caregivers, community partners, office staff, and other key stakeholders.

The care manager will manage a panel of clients and report progress regularly to contracted Medi-Cal Managed Care Plans (MCPs) and complete documentation as required by the MCPs for coordination and billing purposes. The ideal candidate should possess a commitment to working with the unhoused, experience working with this population, outstanding communication skills, prior case management experience, exemplary collaborative skills, cultural humility, and organizational ability to manage a panel of at least 15-25 patients.

Salary Range: $29.64 - $39.13 HourlyQualifications

Required:

  • Bachelor's degree in social work from program approved by the Council on Social Work Education OR equivalent combination of education/experience

  • BLS from the American Heart Association or American Red Cross

  • Must possess a valid California Drivers' License for the class of vehicle to be operated and must have no license restrictions other than for corrective lenses.

  • Position is subject to the California DMV's "Pull Notice System" and continued employment is contingent upon proof of a satisfactory driving record.

  • Minimum 1 years of relevant experience.

  • Minimum of 1 year working with the unhoused or similar populations

  • Prior experience with population management and/or care coordination.

  • Experience working with people experiencing homelessness.

  • Excellent communication skills; verbal, written, and interpersonal.

  • Computer skills in Word, Excel, PowerPoint, and Outlook

Preferred:

  • Experience with Whole-Person Care or ECM work.

  • Experience in street medicine or community-based care for people experiencing homelessness.

  • Background in Healthcare Administration, Business, Public Health, or a closely related field.

  • Experience with insurance eligibility checking, prior authorizations, referrals, and orders for government payors.

  • Ability to be flexible with working hours given adequate notice and ability to work weekends and evenings as required.

  • Experience documenting in an EMR.

  • Lived experience of homelessness

  • Bilingual (English/Spanish) language proficiency
