The Clinical Supervisor for Care Management oversees the day to day activities of the Care Management Department. The Clinical Supervisor provides support and guidance to the Care Management Staff. This will involve balancing clinical, strategic, regulatory, risk management and financial aspects in everyday decisions about care coordination, and resource utilization and discharge planning. The Clinical Supervisor role also encompasses education of the entire interdisciplinary team on the goals of the Care Management Department. The Clinical Supervisor works collaboratively with other departments to achieve positive outcomes and a positive patient experience for all St. Francis patients and families. Job Details Job responsibilities Establishes, manages, and evaluates day to day activities of the Care Management Department. Maintains and oversees Care Managers daily assignment to ensure adequate staffing on patient care units. Processes time off requests and schedule. Provides consistent and constructive feedback to Care Managers on their performance. Provides recognition and rewards the achievements of other on an ongoing basis. Utilizes conflict resolution and negotiation skills to ensure timely resolution of issues. Refers cases and issues to the Director or Assistant Director of Care Management in compliance with identified procedures and follow-up as indicated. Identifies, plans and coordinates aspects of care for the patient/family. Initiates required forms and or letters to comply with regulatory guidelines. Completes timely and appropriate documentation. Position Requirements Graduate of an accredited Nursing School or College; current New York State R.N. License required, BSN Bachelor of Science in Nursing or Health, Master Degree in related field preferred. Experience in acute care Care Management Approved National Certification preferred Management experience in Care Management preferred Basis knowledge of personal computer and software for word processing and spreadsheets and/or Hospital Information Systems preferred, or the ability to enter and retrieve data from relevant computer systems. Salary Range USD $49.65 - USD $74.76 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
