Registered Nurse

Under general supervision, provides comprehensive health services as applicable. Works collaboratively with Clinic and outside providers to facilitate client wellness. Maintains all applicable confidentiality and ethical standards of relevant governing bodies. Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma informed care environment.

  • Provides a comprehensive health assessment of clients.

  • Identifies immediate and long-term health care needs.

  • Makes appropriate referrals and recommendations based on findings of the health assessments.

  • Assists clients in identifying local resources.

  • Communicates and collaborates with appropriate Agency staff, and related community organizations to assure coordination of client care and services within the Connections To Care program.

  • Completes required paperwork / electronic patient record forms related to the health assessment process.

  • Coordinates care with primary care providers and other specialists through health monitoring and complex care services

  • Assists with program planning, design and implementation of Connections To Care program with the Nursing Services Manager, Associate VP of Nursing.

  • Provides smoking cessation services for individuals and groups.

  • Participates as assigned by Nursing Supervisor in community-based outreach and education programs.

  • Assists with preparation of needs assessments and collaborates with Nursing Manager to develop training content for CCFCS staff with employment in patient facing positions within the OPWDD, OMH and OASAS programs. (Connections To Care)

  • Evaluates program effectiveness and recommends alternatives as required.

  • Assists Prescribers

  • Provide health monitoring for clients

  • Participates in regular Case Conferences

  • Administers Injectable medications as prescribed

  • Responsible for patient records

  • Completes required documentation in accordance with regulations and other record keeping as necessary for program accountability

  • Participates in quality assurance and program evaluation activities as directed.

  • Provides education to clients on health and wellness issues.

  • Participates in staff meetings and clinical supervision as directed

  • Actively pursues development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops.

  • Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan. Including, but not limited to, supporting the Agency's Core Competency initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse

  • Participates in relevant Agency meetings and/or trainings

  • Other duties as assigned

Qualifications

Minimum of an Associate in Applied Science degree in Nursing required. Bachelor of Science Degree is preferred.

Professional Registered Nursing License in good standing.

Experience: At least two years of experience related to nursing services to individuals with Intellectual Disability, Mental Health and Substance Use Disorder related services strongly preferred. Bi-lingual in Spanish also preferred.
