Community Service Administrative Assistant - 15.0 hours/week

Pay Range: $17.80 - $20.00

The Community Service Administrative Assistant performs QA investigations, maintains incident databases, and clerical duties related to quality assurance and audits for Service Coordination. This position will also assist with community referrals for the Supportive Housing Program and help assist the potential resident complete the intake process. This position will educate the community on services with guidance from the Director of Supportive Housing. The Community Service Coordinator will be responsible for gathering data for internal and external reports and conducting audits to ensure compliance with ESLC and Dept. of Health standards and regulations.

ESSENTIAL JOB FUNCTIONS

  • Conduct Quality Assurance Investigations including, but not limited to, Internal Recordable Incidents and Serious Reportable Incidents.

  • Call and schedule interviews for applicable investigations.

  • Ensure all complaints and resolutions for the agency are documented on the Complaint Log within 14 days.

  • Prepare and send Quality Assurance Community Services Department specific satisfaction surveys annually and perform data collection with analysis.

  • Collaborates with Vice President of Community Services to find and apply for grant opportunities to support community programs

  • Tracks and reports necessary information/data to fulfill any grant requirements.

  • Provides program development support based on market trends, needs and interests, and ESLC values.

  • Prepare and file Serious Reportable Incidents (SRIs), including during non-business hours, as applicable.

  • Collect data from appropriate staff to complete occupancy report, Minority and Women owned business reports, Health and Wellness report tracking, Health and Wellness initiative reports, neighborhood program reports, and other reports essential to maintain the integrity of the Supportive Housing program and meet DOH and ESLC standards.

  • Collect and analyze data for quarterly reporting to various departments/external offices.

  • Record meeting minutes for the QA Department.

  • Complete chart audits, reporting outcomes to Director and/or Manager

  • Complete phone screening of potential program participants and then reporting outcomes to Director and/or Manager

  • Maintain referrals and waitlist for eligibility assessment in Supportive Housing

  • Partner with Director of Supportive Housing to attend functions for referral sources and /or program information.

  • Document in-person and telephone encounters with potential participants and referral sources

  • Complete required documentation

  • Perform other duties as assigned.

Requirements

EDUCATION:

  • Associate Degree in Health and Human Services Field or three years of equivalent experience.

QUALIFICATIONS:

  • Experience with Microsoft Office Suite required.

  • Possess a current, valid New York State driver’s license and a reliable vehicle.

  • Experience preferred in workplace investigations.

  • Must have strong organizational skills.

  • Must maintain confidentiality with sensitive information.

  • Must be detail oriented.

  • Past clerical experience preferred.

  • Strong communication skills.

  • Ability to effectively engage patients in a therapeutic relationship, when appropriate.

  • Experience working with Program Participants who have co-occurring mental health, substance abuse, and physical health problems, preferred.

  • Experience with community based and other social support services as well as healthcare services that respond to the individual’s needs and preferences and contribute to achieving the individual’s goals, preferred.

WORKING CONDITIONS:The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. The majority of essential job duties are performed indoors, in a normal office environment. Being located at the main entrance of the building may expose the employee to drafts and temperature fluctuations.

SUPERVISORY RELATIONSHIP:The work is performed within established guidelines, and requires periodic supervision. The employee exercises some initiative and independent judgment to effectively perform the essential job duties of this position.RESPONSIBILITY FOR OTHERS:The employee has no direct responsibility for others.

SAFETY RESPONSIBILITIES:Shall be fully acquainted with all safety policies and procedures of ESLC. Takes an active role in all fire drills during the shift and/or emergencies for internal and external disasters as monitored by supervisor and incident manager. Must comply with facility in-service requirements.
