Humana Equitable Population Health Management (EPHM) Director in Richmond, Virginia

The Equitable Population Health Management (EPHM) Director (Director, Health Equity Strategy) is responsible for setting direction and establishing strategy to advance equitable health outcomes for our members in Virginia. To achieve this, the EPHM Director will work with Providers and community-based organizations to improve HRSNs and health outcomes for our membership. The EPHM Director will also focus on fully integrating culturally informed strategies into the design and development of clinical and population health programs, services, and communication. The EPHM Director will work directly with the VA CEO and other functional area leaders to ensure alignment across the market.

  • Lead in developing and implementing strategic initiatives to support an organizational workforce and culture that is inclusive and respectful of the various backgrounds and cultures that are reflected in our members and communities we serve.

  • Lead a team of professionals focused on community engagement, member education and advocacy, population health evaluation and analytics.

  • Drive strong relationships with communities we serve and community groups to identify racial, linguistic, gender and geographic health disparities, while developing and implementing strategies to increase equity for our members.

  • Listen to, and give voice to, emergent issues of equity within the local health care community and the impacts on healthcare quality, health care cost and access to care for historically underserved communities.

  • Build and maintain sustainable strategic relationships with community partners, state agencies, and providers.

  • Provide leadership for community involvement and develop/foster alliances and partnerships to strengthen Humana Healthy Horizons in Virginia's equity-centered population health approach.

  • Influence external and internal audiences on critical population health and health equity issues.

  • Partner with clinical and community leaders to implement equity-focused community health strategies and initiatives that improve health outcomes.

  • Create and drive evidence-based, scalable, and financially sustainable population health solutions.

  • Develop, Monitor and analyze Indiana health equity, antiracism and population health performance and progress metrics to gauge effectiveness of initiatives.

  • Works with plan executive leadership and their teams to develop synergies around health equity related to emerging or current DMAS and/or Medicaid priorities.

  • Serve as a trusted thought partner in health equity with plan leadership, local communities, and state agencies.

Required Qualifications

  • Must reside in the Commonwealth of Virginia.

  • Bachelor's degree.

  • 8+ years of experience effectively leading programs that advance improved clinical and population health outcomes.

  • Experience working directly in communities and understanding health disparities and barriers to health equity.

  • Experience as a leader who clearly communicates with and engages community leaders, including the clinical community, to build trusted relationships, enhance the connection to a company, and/or engage in partnerships or initiatives to improve health.

  • Demonstrated ability to operationalize a vision and plan, organize and integrate priorities and deadlines.

  • Demonstrated ability to drive change in alignment with the mission, vision and values of an organization.

  • Strong understanding of analytics and demonstrated ability to translate insights into business strategy.

  • Excellent interpersonal skills, ability to develop effective relationships with a broad array of people (different levels of management and clinical expertise).

  • Experience with program planning, implementation, and evaluation.

  • Engages experts and stakeholders in the field of equity, antiracism, and population health to develop and maintain a forward-thinking culture of continuous improvement and learning.

  • This role is a part of Humana's Driver Safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits.

Preferred Qualifications

  • Advanced degree (M.D., PharmD, RN, NP, PA, PhD, MBA, MPH, MPP, or MPA)

  • 5+ years of experience in leadership positions addressing issues related to disparities, inequity, racism, and social justice.

  • Ability to analyze data and make data-driven recommendations for quality improvement.

  • Demonstrated experience leading equity efforts in a complex organization and industry resulting in measurable outcomes.

Additional Information

  • Workstyle: Hybrid Home - The associates home office environment is their primary work location where most of their work week will be spent, but they will use Humana Healthy Horizons in Virginia office space in Glen Allen, VA, occasionally for collaboration and other face-to-face needs.

  • Typical Workdays & Hours: Monday - Friday; 8am - 5pm EST

  • Travel: Up to 10% throughout Virginia to meet community leaders/providers to develop strategy and programs to address health equity.

To ensure Home or Hybrid Home/Office employees' ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria:

  • At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is recommended; wireless, wired cable or DSL connection is suggested.

  • Satellite, cellular and microwave connection can be used only if approved by leadership.

  • Humana will provide Home or Hybrid Home/Office employees with telephone equipment appropriate to meet the business requirements for their position/job.

  • Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.$149,800 - $206,100 per yearThis job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
