The Community Health Hub - Program Manager

not a remote working position

Hours: Mon-Fri with occasional weekends

The Program Manager of Community Health will oversee the ongoing operations and optimization of current and future community health programs in the Market. The Program Manager will help initiate and mature programs and partnerships aligned to the Community Health Needs Assessments (CHNA) and Implementation Plan (s) ensuring it meets IRS requirements, industry best practices and is a collaborative endeavor including community partners, community members with special knowledge of underserved populations and internal stakeholders. The Program Manager will develop outcomes measurements, metrics, and appropriate instruments to evaluate (program impact, and collect data and program narrative to showcase success of community initiatives and promote community health investments).

Requirements:

  • Bachelors Degree in Social work, public health or a health services field, public policy, public administration, community development, or social ethics. (Required)

  • Masters Degree in Public Health, Public Policy (Preferred)

  • 5 years experience in program development / project management (Required)

  • Bilingual skills strongly preferred.

  • Strong analytical and communication skills, effective interpersonal skills, previous management experience, ability to build effective teams, innovative thinker, proficient in technology including Microsoft office.

Essential Job Functions:

  • Collaborate in the planning and execution of annual and long-term work plans including participation in the development and implementation of project/program objectives.

  • Develops and maintains robust measurement and monitoring of community health programs.

  • Develops and maintains effective professional relationships with internal and external stakeholders including community leaders and residents across diverse interests, cultures, socio-economic backgrounds, and populations.

  • Maintains awareness of industry trends, community conditions, associations, and activities in the Market to organize and leverage for community initiatives.

  • Partner with Director of Community Health Programs and Community Outreach Manager to meaningfully incorporate and expand key Market programs.

  • Prepare quarterly reports with evaluation metrics on programs.
