Patients are often admitted for acute inpatient rehabilitation with the following conditions or diagnosis: amputation, brain injury, cardiac or pulmonary conditions, hip fracture, neurological conditions, orthopedic conditions, sports injuries, stroke, spinal cord injury, or traumatic injury/fractures. St. Charles Hospital is the only Long Island hospital accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF) for comprehensive inpatient rehabilitation and outpatient medical rehabilitation for adults and children. Job Details Our Rehab Admissions Nurse Liaison (RN) will: Evaluate patients for admission to the St. Charles Inpatient Physical Rehabilitation Program Travel and visit with patients currently admitted at other local hospitals as well as on the St. Charles main campus, and assess for admission into the inpatient rehab program Educate patients and family members about inpatient rehab Determine medical stability and readiness of the patient for transfer into the rehab program Coordinate a smooth transition from the referring facility to the rehab setting Partner with an interdisciplinary team including, but not limited to, attending physicians, Registered Nurses (RNs), and ancillary staff to optimize patient outcomes and satisfaction. Advocate for and build meaningful relationships with patients and their loved ones through safety intervention, emotional support, and education. What you’ll bring: Passion for Rehab Nursing 5+ Years of clinical hospital based RN experience required Previous Subacute Rehab experience also preferred Education: Graduate of an accredited School of Nursing Bachelor of Science in Nursing (BSN) degree preferred Licensure: New York Registered Nurse (RN) License & Registration Salary Range USD $57.56 - USD $60.50 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience.
