Job Description:

Duties/Responsibilities:

  • Completion of psychosocial assessment of patient.

  • Development of case plan with patient, family and medical interdisciplinary team.

  • Utilize appropriate interventions to address current and/or future problem; facilitate referral to support services.

  • Provide consultation to other disciplines and staff on biopsychosocial, environmental and cultural factors.

  • Brief therapeutic intervention with patient and family around the issues of diagnosis, adjustment, grief or loss.

  • Formulation and implementation of support services to provide continuum of care for patients and families.

Education/Experience Required:

  • Two years of medically-related Social Work.

Registration/Certification:

  • M.S.W.- Required

  • LCSW/LICSW

Physical/Environmental Requirements

This job requires frequent bending, carrying, pulling, pushing, reaching, standing, stooping, walking and lifting over 10 lbs. There will be occasional need for climbing stairs, lifting in excess of 50 – 100 lbs and sitting. Constant use of hearing, sight and speech. Frequent use of smell and touch. Frequent keyboard and telephone use. Constant concentration and problem solving.
