Trinity Health Life Sharing Program Specialist in Pittsburgh, Pennsylvania

Position Purpose:

The Life Sharing Program Specialist provides guidance and support to individuals with intellectual disabilities and autism in a provider home and Provider Family. The Program Specialist position requires monthly visits and communication with person served and provider family. This position requires strong organizational, decision making, and problem-solving skills and the person must be capable of working independently.

What you will do:

  • Use independent judgment when providing assistance and support to individuals families.

  • Manage daily schedule to achieve and maintain program specific and organizational established standards or productivity and output

  • Complete or correct and review all documentation on time as required according to applicable regulations, agency and program policy and procedures. This includes medical appointment summaries, cash on hand ledgers, Social Emotional Environmental plan, assessments, etc.

  • Express thoughts, expectations, ideas, and intent effectively and concisely, orally and in writing.

  • Maintain confidentiality according to Organizational Policy and Procedures.

Minimum Qualifications:

  • A master’s degree or above AND 1 year of work experience working directly with individuals with an intellectual disability or autism

  • OR a bachelor’s degree AND 2 years of work experience working directly with individuals with an intellectual disability or autism

  • OR an associate degree or 60 credit hours from an accredited college/university AND 4 years of work experience working directly with individuals with an intellectual disability or autism.

  • Must have a valid driver’s license and daily access to a vehicle.

  • Act 33/34/73 Clearances

  • Pre-employment Drug Screen, Physical/TB
