Description:

The Patient Care Manager (PCM) is a Registered Nurse who is responsible and accountable for the coordination of all clinical activities and daily operations on the unit for a 24 hour period. He/she works in conjunction with the staff to coordinate the care of all patients on the unit. The PCM coordinates daily allocation of resources and promotes the development of work teams. He/she works under the guidance of the DON.

  • Participates in the formulation, revision and advancement of department strategic short and long-range plans in collaboration with the Director of Nursing.

  • Directs the development and implementation of unit-based operational plan to ensure the optimal functioning of assigned nursing unit(s) and the facilitation of Hospital operations.

  • Manages daily operations within assigned unit(s), including staffing, clinical practice, resource utilization, quality management, and personnel management.

  • Responsible and accountable for coordinating the assessment, planning and implementation and evaluation of all actives directed towards meeting the nursing care needs of the patients served.

  • Ensures effective care coordination, including patient flow and that hand-off communication is aligned with SBAR model.

  • Systematically examines and participates in the revision of clinical nursing practices and nursing standards of care in accordance with overall nursing standards of care, changes in patient populations, as well as external standards.

  • Works collaboratively with other disciplines to ensure comprehensive patient-centered, outcome driven approach to care. Identifies and recommends approaches to improve efficiency and effectiveness of patient care and placement from admission through discharge

  • Maintains assignment records as per record retention policy.

  • In collaboration with staff, establishes a clearly defined process to ensure patient care assignments are based upon staff competency and patient care needs

  • Maintains clinical skills through participation in direct patient care activities.

  • Participates in the evaluation of clinical competency review of staff and provides recommendations for retention and promotion.

Master’s degree is required and may be in Nursing, a related health care field or Business Administration. One degree, either Bachelor's or Master's, must be in Nursing. If currently pursuing and making steady progress toward completion of a master’s degree, completion must be within 24 months of hire.

Three (3) years in acute clinical nursing preferred. Previous leadership roles and experiences preferred.
