Schedule: Full-time

Shift : Day shift, rotating weekends

Location: field work position in homes and hospitals within Volusia and Flagler counties

The role you’ll contribute:

The Admissions Nurse is responsible for responding to and visiting with patients who have been referred for admission to AdventHealth Hospice Care. The Admissions Nurse informs the patients, caregivers, and family members of the hospice services available with the organization and assesses patient for admission to services. In addition, the Admissions Nurse is responsible for visiting current AdventHealth Hospice Care patients who have been admitted to the hospital and communicates patient status with the respective interdisciplinary team.

The value you’ll bring to the team:

· Provides information and resources available to patients for admittance onto Hospice services.

· Travels to resident site or hospital room of patient who has been referred to Hospice. Greets patient, caregiver and/or family member(s) and communicates the program and services available with Hospice.

· Partners with the Account Representative staff member to meet the needs of the patients and hospital staff to gain new referrals.

· Assesses the patient and family’s emotional condition as to readiness for hospice. Answers questions and responds to concerns to encourage admission onto the Hospice program if conditions are justified.

· Obtains required signatures and generates handwritten documents required to comply with regulations for reimbursement.

· Assesses the patient’s physical condition, medical needs, current medications, and needs for special equipment. Determines patient’s primary physician, personal information, and special needs. If patient’s condition requires crisis intervention, psychosocial, chaplain or other special services, notify Hospice of need to assign appropriate team members. Obtain orders and initiate documentation for crisis care if required.

· Appropriately evaluates hospice eligible patients in a timely manner. Obtains approval from Hospice physician and the patient’s primary care physician to admit client to program. Communicates with the patient’s primary care physician with notification regarding admission of patient and care plan initiation with review.

· Promptly updates referral office/sources on the status of admissions.

· Documents information received from pre-admission visit to gather basic patient information and enters into the electronic medical record in a timely manner.

Qualifications

The expertise and experiences you’ll need to succeed:

EDUCATION AND EXPERIENCE REQUIRED:

· Associates Degree.

· Minimum of 3 years Registered Nurse experience in home health and/or acute care nursing experience; including 1-year experience in the hospital

· An equivalent combination of education and experience may be substituted at the discretion of the Administrator

EDUCATION AND EXPERIENCE PREFERRED:

· Bachelor of Science in Nursing.

LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:

· A clear/active Florida Registered Nurse license.

· Basic Life Support Certification.

· Maintain a current, valid Florida Driver's License in good standing with a safe driving record and a reliable vehicle.

· Maintain valid personal automobile insurance.

LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:

· Certified Hospice and Palliative Nursing certification.
