Shift : M-F 8-430 ( Hybrid – in the beginning it will be 4 days in office 1 at home and eventually move to 2 days at home)

The role you’ll contribute:

The Clinical Program Manager, in collaboration with medical directors and campus leadership, are responsible for the coordination of clinical excellence in care within the assigned service line across the continuum of AdventHealth. The manager functions as a key decision maker for the interdisciplinary team, which is responsible for monitoring and improving the assigned service line’s clinical program performance as well as regulatory compliance and accreditation requirements. The manager facilitates the development and implementation of the clinical program’s educational initiatives for staff and patients. The manager leads campus interdisciplinary teams in performance improvement and the incorporation of evidence-based practice protocols into the daily operations of the clinical program. The manager is responsible for analyzing outcomes to determine root causes of variation and developing corrective actions to improve outcomes and performance. The manager serves as a resource for inpatient and outpatient team members and providers and is responsible for documentation of appropriate information in the patient’s medical record. The manager participates in data management and review, identifies adverse trends and reports on them as well as provides conclusions and recommendations based on this data. The manager will also maintain oversight over required regulatory and program specific data. The manager actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

· Responsible for serving as the clinical program content expert through skilled knowledge of clinical evidence-based guidelines, national accreditation standards and regulatory (state and federal) program requirements specific to the assigned clinical service line. Responsible for maintaining knowledge of current trends and clinical practice by continuing education programs and review of current evidence-based literature.

· Leads the interdisciplinary committee(s), in partnership with the service line medical director to improve clinical performance, standardize clinical processes, ensure compliance with accreditation and regulatory standards. Responsible for identifying variations in care and leads the development of corrective action plans to improve performance. Leads committees (as assigned) to review clinical opportunities to improve clinical outcomes. Leads quality assurance performance improvement (QAPI) review meetings, and attends any other service line committee meetings, as appropriate.

· Leads the initial and ongoing national accreditation process by leading readiness rounds, education, and maintaining documentation to meet all accreditation standards. During accreditation site visits serves as the clinical program lead for the campus, including formulation of any corrective actions, in collaboration with the VP and/or Medical Director of the clinical program to maintain accreditation.

· Responsible for the clinical development and ongoing review of evidence-based clinical pathways to standardize the assigned service line’s clinical care across the continuum. Continually reviews clinical guidelines and updates clinical pathways to ensure delivery of evidence-based care. Responsible for leading stakeholder and interdisciplinary committee approval process for timely implementation and updates of clinical pathways (as assigned).

· Responsible for continuously evaluating the clinical program’s metric performance, ensuring accurate public reporting of process and registry measures, as needed. Responsible for maintaining accurate records and data regarding the clinical program as required by the service line to meet regulatory requirements. Ensures data accurately reflects clinical care provided, shares outcomes routinely with committees and clinical campus leadership. Responsible for providing real-time feedback to campuses on the clinical program’s patients and processes to drive performance improvement.

· Responsible for maintaining a professional work environment that organized and structured to achieve job responsibilities. Responsible for ensuring all patient information is protected per privacy policies and HIPPA regulations

Qualifications

The expertise and experiences you'll need to succeed:

KNOWLEDGE AND SKILLS REQUIRED:

· Advanced knowledge of the assigned clinical service line including nursing experience in the service line.

· Ability to work in a complex matrix relationship

· Strong analytical and communication skills

· Strong interpersonal skills, ability to build relationships with various disciplines that foster trust

· Strong leadership skills including public speaking, team building, staff development, communication

· Strong understanding of complex pathophysiology and surgical procedures to make clinical judgements on appropriateness of clinical care to measure quality and outcomes

· Knowledge of state/federal regulatory agencies, accreditation standards and national society clinical guidelines

· Strong critical thinking skills and ability to balance multiple competing priorities

· Ability to mentor, enhance and engage organizational talent

· Strong organization skills to handle multiple priorities and projects; able to manage financials, budget, clinical, service, and team to meet goals in an organized, prioritized fashion

· Effective oral and written communication skills with the ability to articulate complex information in understandable terms to all levels of staff

· Competence in the latest computing software including but not limited to Microsoft Office

EDUCATION AND EXPERIENCE REQUIRED:

· Graduate of school of nursing (RN)

· Bachelor’s Degree in Nursing, Healthcare Administration or other healthcare related discipline

· 5 years nursing experience

· 2 years progressive leadership experience

LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:

· Current, valid State of Florida license as a registered professional nurse (RN)
