Program Coordinator - Community & School Health Initiative

Northridge Hospital Medical Center

We are seeking a highly motivated and experienced Program Manager to lead our community and school health initiative. The Program Coordinator is responsible for coordinating, supervising, implementing and evaluating programs, services and activities for the Center for Healthier Communities for multiple grant funded projects. These programs focus on areas including chronic disease management and prevention and abuse prevention programs. This position is also responsible for program development and grant writing.

Responsibilities

  • Responsible for the development and implementation of CHC grant funded programs including the California Department of Public Health Prevention Forward grant and California Office of Emergency Service Sexual and Domestic Violence Prevention Grant.

  • Supervises achievement of deliverables and leads team members for the CHC programs.

  • Facilitates workshops classes and groups to educate community members for CHC programs.

  • Develops effective outreach programs for CHC programs.

  • Builds capacity by creating new partnerships with other community based organizations.

Qualifications

  • Experience managing community based programs abuse prevention ability to create multi-disciplinary team grant reporting and project management.

  • Experience creating and presenting workshops and evidence based curriculums.

  • Bachelor Degree (BA/BS) required.

  • Fire Safety card required within 90 days of hire.

Pay Range

$29.03 - $42.10 /hour
