Job Title: Licensed Practical Nurse Care CoordinatorJob Description
As a Licensed Practical Nurse Care Coordinator, you will play a crucial role in preparing patients for their visits, ensuring all necessary information such as chief complaints, medication lists, and allergy records are up-to-date. You will be responsible for maintaining the cleanliness and readiness of exam rooms, sterilizing instruments, and managing medical equipment. Your role includes documenting interventions in the electronic medical record, ensuring completeness of the patient's chart, and obtaining any additional medical records required prior to patient visits. You will also handle patient communications regarding prescription refills, lab results, and follow-ups, as well as collect patient satisfaction surveys and monitor patient medical status between visits. Adhering to protocols related to infection control, environmental safety, and patient confidentiality is paramount.
Hard Skills
Practical Nurse License
BLS Certification
Experience in Ambulatory and Diabetes/Endocrinology (preferred)

Work Site
This is a fully on-site position in Norfolk, Virginia.
Work Environment
Outpatient office setting with a Monday to Friday, 8-5 schedule. No weekends or holidays required, with minimal overtime.
