Job Overview:

The Advanced Practice Nurse Provider the Transitions of Care Heart Failure Program works collaboratively within a multidisciplinary health team and is responsible for providing comprehensive and age appropriate direct care and follow up to a population of patients. The components of the role include clinical practice, consultation, professional development, education, and research. The APN works in conjunction with the house staff performing patient consults and follow up care. Clinical core privileges will include the management,

Qualifications:

Required:

  • National Nurse Practitioner Certification

  • Active NJ APN license

  • Active NJ RN license

  • Graduate of an accredited APN program

Certifications and Licenses Required :

  • National Nurse Practitioner Certification

  • Active NJ APN license

  • Active NJ RN license

Essential Functions:

    1. Actively identify patients that require heart failure management and treatment and work collaborately with admitting attending to coordinate care
    1. Assess, examine, evaluate and provide treatment for patients with acute and chronic heart failure in both inpatient and outpatient setting
    1. Coordinate post acute care needs with case management and social work to ensure safe discharge and care transitions
    1. Provide follow up phone follow up post discharge to monitor achievement of clinical outcomes and communicates with pimary and specialy teams regarding unanticipated variances
    1. Assesses complexity of care needs and potential/actual issues or gaps in care
    1. Identifies, tracks, and conducts root cause analyses on readmissions to address programmatic and system-wide improvements
    1. Identifies patient/family education needs and ensures that patient/family members have adequate information to participate in transition planning
    1. Participates in clinical performance improvement actitivies to achieve set goals
    1. Develops, implements, and evaluates comprehensive patient education programs that assure quality and appropriateness of care across settings (i.e. inpatient, post-discharge, and home).
