Job Overview:

RWJBarnabas Health is seeking a Nurse Practitioner or Physician Assistant for a dynamic position involved in the care of Kidney and Pancreas Transplant patients.

Qualifications:

Required:

  • Certified Nurse Practitioner licensed in the state of New Jersey

  • Registered Nurse in the state of New Jersey and/or Physician Assistant PA license

  • Basic Life Support BLS

Preferred:

  • Certified Clinical Transplant Coordinator CCTC certification preferred

  • Experience with ESRD population and /or transplantation is preferred

Certifications and Licenses Required:

  • Certified Nurse Practitioner licensed in the state of New Jersey

  • Registered Nurse in the state of New Jersey and/or Physician Assistant license

  • Basic Life Support BLS

Essential Functions:

  • Clinical responsibilities inclusive of transplant candidates, living donors, post-transplant patients and patients enrolled in research trials

  • Obtains patient histories and perform physical examination

  • Ensures continuity of care through effective communication and collaboration with the multi-disciplinary team in all areas of the transplant department

  • Provides consultation, education, and referral services for transplant candidates, transplant recipients and living donors

  • Performs clinical duties related to scope of practice including such things as staple removal; wound care; adjustment of immunosuppression; review of recorded BP and adjustment of anti-hypertensives accordingly; monitoring weight; adjusting diuretics/monitoring electrolytes etc

  • Acts as a liaison between transplant coordinators, transplant physicians, transplant consultants and ancillary staff in providing continuity of care to patients and families

  • Participates in the development of appropriate clinical protocols and related policies and procedures

  • Participates in patient care conferences including weekly Case Review meetings, Transplant Grand Rounds; Transplant Research Meeting; Transplant Candidate Review Board and Journal Club

  • The primary role will be inpatient duties but may rotate into the outpatient clinic as needed

  • Responsibilities include but not limited to: admissions, consults, rounding on patients and discharges in collaboration with attending MD; ordering necessary testing and reviewing results; education to staff, patient and families; evaluating, consenting, and presenting at selection meeting; participation with regulatory compliance UNOS, CMS, & TJC; participation in QAPI activities as assigned

  • Must be a motivated and independent thinker, able to quickly prioritize, work in a fast-paced, highly clinical level environment and adapt quickly in acute situations
