Description:

To plan, organize, develop, and direct the overall operation of the Social Services Department in accordance with

current federal, state, and local standards, guidelines, and regulations along with established policies and procedures

and as may be directed by the Administrator, to meet medically-related emotional and social needs of the resident.

ENTIAL FUNCTIONS

  1. Know, understand, incorporate, and demonstrate the Mission, Vision, Values and Guiding Behaviors of Trinity

Health and Trinity Senior Living Communities in behaviors, practices, and decisions.

  1. Consistently demonstrate (leads by example) Sanctuary Values to all internal and external customers (residents,

visitors, volunteers, and associates.) Create and maintain an atmosphere of warmth, propagating a calm

environment throughout the community. Actively participates / intervenes with direct care as needed and as a role

model in ongoing Sanctuary Training of personnel.

  1. Execute assignments in a culture that is shared and collaborative across all divisions within TSLC.

  2. Establish and/or maintain departmental procedures that support the necessary operational functions for the Social

Services Department.

  1. Maintain department budget, place orders for equipment and supplies as necessary.

  2. Determine departmental staffing requirements and assist in the recruitment, interviewing, and selection of

personnel for the Community or Department.

  1. Monitor department work hours (including vacation and holiday schedules), personnel (if applicable), work

assignments, etc., to ensure proper workflow and adequate staffing levels in compliance with state minimum

regulations and sanctuary budgeted PPD hours.

  1. Evaluate, develop, mentor, coach, counsel and discipline department staff. Supports community personnel from

other departments through coaching and mentoring to help achieve optimum standard of excellence. Addresses

issues of concern through courageous conversation and notifies department manager of any interactions requiring

attention.

  1. Participate in community surveys and when necessary develop a plan of correction for department deficiencies.

  2. Assume the authority, responsibility and accountability of directing the social service department and its programs.

  3. Participate in planning related to resident, family, and community interests. Participate in discharge planning,

development and implementation of social care plans and resident assessments. Perform administrative duties such

as completing medical forms, reports, evaluations, studies, charting, arranging transportation, etc., as necessary,

coordinating activities with other departments as necessary.

  1. Promotes optimum transition of resident for appropriate and safe placement for discharge. Provides ongoing

communication with resident and responsible persons of progress/status during stay.

  1. Provide information to the resident/family on Medicare/Medicaid as well as other financial assistance programs

available. Provide consultation to staff, community agencies, etc., in efforts to solve resident concerns through the

development of social service programs.

  1. Keep abreast of economic conditions/situations, recommending adjustments that assure continued provision of

daily social services.

  1. Assure social service progress notes are informative and descriptive of the services provided as well as the

resident's response(s) to the service. Review and develop a plan of correction for social service deficiencies.

  1. Provides information and education for residents and responsible person on Advanced Directives to support

informational needs. Collaborate with Interdisciplinary team related to advance directives and assures medical

records are compliant with the completion.

  1. Serve on, participate in, and attend various committees of the community (i.e., Infection Control, Policy Advisory,

Pharmaceutical, MDQI, etc.) as required or appointed by the Administrator/Executive Director.

  1. Provide written and/or oral reports regarding social service programs and activities as directed by such

committee(s). Conduct grievance tracking and report out in the appropriate meeting. Evaluate and implement

recommendations as they may pertain to social services.

  1. Participate in resident/group council meetings as requested and provide support services to such council.

  2. Forecast needs of the department. Assist in preparing and planning the social services department budget, making

adjustments as necessary. Maintain current written records of department expenditures and cost reports.

  1. Respect residents’ rights, including maintaining the confidentiality of all resident care information, knocking before

entering a resident's room, and ensuring all social services personnel are knowledgeable of the residents'

responsibilities and rights including the right to refuse treatment. Review resident complaints and grievances,

making written/oral reports indicating what action(s) were taken to resolve them.

  1. Investigate and submit accident/incident reports to Human Resource within 24 hours after occurrence.

  2. Must function independently, make independent decisions, demonstrate flexibility, personal integrity and ability to

work effectively with residents and personnel.

  1. Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance

with the community policies and procedures.

  1. Maintains a working knowledge of applicable Federal, State, and local laws and regulations, TSLC Corporate

Integrity Program, Code of Ethics, as well as other policies and procedures in order to ensure adherence in a

manner that reflects honest, ethical and professional behavior.

  1. Provide residents with information concerning resident rights, living wills, etc., explaining as necessary. Serve as

grief counselor to families in need.
