Position Summary:

The Director of Community Health & Well-Being (CHWB) works in collaboration with CHWB Vice President for the Region to ensure that the region is fulfilling its commitment to promote optimal health for those populations who are poor and vulnerable and communities we serve by connecting social and clinical care, addressing social needs, dismantling systemic racism and reducing health inequities. This position sets strategy, leverages resources and partnerships focusing on those who are poor and vulnerable, and addressing the social influencers of health. This position provides oversight of community health programs and personnel, and acts as a liaison to community agencies in determining community needs. In addition, this position is accountable for CHWB system scorecard measures contributing to community benefit reporting, and progress toward achievement of the CHWB balanced scorecard.

What the Director of Community Health and Well-Being will do:

  • Directs personnel, operational and financial activities over multiple hospitals, inclusive of, community health workers and other CHWB positions. Responsible for making decisions and managing risk that impact the functional area or program operations.

  • Oversees staffing and succession planning activities, develops, coaches, mentors and motivates colleagues. Fosters and sustains a challenging, inclusive and rewarding work environment where talent is developed and diversified. Facilitates diverse member interests, teams and groups through strong leadership, communication, collaboration and negotiation abilities to mutually agreeable resolutions. Ensures compliance with established Hospital and personnel policies when operating the Department.

  • Develops long and short-term goals, objectives, plans and budgets for all programs within CHWB to ensure continued quality of services through planning across the region with other functional areas.

  • Serves as a CHWB knowledge resource and convener of stakeholders, representing Ministry's commitments to community health improvement and partnerships.

  • Oversees and develops measureable community health improvement plans and implementation strategies, in collaboration with Ministry senior leadership, that align with the community health needs assessment and Trinity Health strategic plan.

  • Identifies and establishes rapport with funding sources such as foundations, state and federal agencies and establishes reporting systems for each funded initiative in order to ensure that programmatic designs comply with all applicable rules and regulations.

What the Director of Community Health and Well-Being will need:

  • Must possess a bachelor’s degree from an accredited college or university with a degree in social work, sociology, psychology or a closely related field. Master's degree preferred.

  • Five years' experience in Community Benefit and Health experience required.

  • Must be able to work independently and exercise a wide range of independent discretion in the performance of their duties.
