Trinity Health Social Worker (MSW) in Merion, Pennsylvania

Shift: Mon to Fri, 8 am to 430 pm, no nights/weekends/holidays

Status: Full time

Location: Merion Station, PA, Philadelphia, PA and Flourtown, PA

Position Purpose:

This position will plan, organize, and implement social services for PACE Organization (PO) participants and families including case management and participant advocacy. Responsible for but not limited to assessment, treatment, teaching and counseling participants, caregiver, or other appropriate representatives.

Position Details:

Position will provide services at 3 locations with services typically will be provided at one location per shift.

The Villa, Sisters of St. Joseph in Flourtown serves as a full-service retirement center for the Sisters of St Joseph.

Manresa Hall in Philadelphia is one of the Jesuit health care communities of the USA East Province.

Mercy LIFE McAuley in Merion Station provides residential and nursing support for the Sisters of Mercy at Merion Station

What you will do:

  • Assess participants, coordinate care plans with IDT.

  • Conduct intake meetings, provide counseling and support.

  • Act as liaison with agencies, provide resource referrals.

  • Coordinate discharge planning and transitional care.

  • Provide individual and family counseling.

  • Act as liaison between the participant and other agencies and facilitate discharge placement planning.

Minimum Qualifications:

  • Master’s degree in Social Work.

  • Two (2) years of experience.

  • One (1) year working with the frail or elderly population strongly preferred.

  • Current social work license preferred.

  • Experience working on a multidisciplinary team in medical/community-based setting preferred.

  • Knowledge of Medicare, Medicaid and other payment systems.

  • Valid driver's license, insurance and use of personal vehicle.

  • Superior written and verbal communication skills.

  • Proficiency with MS Office.

  • Must manage conflict well, have high decision quality and flexibility with ongoing change.
