Responsibilities

Under general supervision to provide personal care including the more complex patient care duties and general housekeeping services for the convalescent clinically ill or disabled; and to do other related work as assigned.

  • Assists patients with more complex health conditions including patients in an unconscious or vegetative state, undergoing IV therapy, nasogastric tube feedings or wound care.

  • Provides personal hygiene including care to the skin, mouth, hair care, and bathing, including showers and bed baths.

  • Provides assistance with ambulation and transfers, including the use of gait belts, Hoyer lifts, walkers and other transfer devices.

  • Provides assistance with prescribed exercises as an extension of therapy services.

  • Prepares light, therapeutic diets, and assistance.

  • Provides assistance with transfers to commodes or the bathroom, or in the use of bedpans or urinals.

  • Provides assistance in the administration of medications which are generally self-administered by the patient.

  • Performs those household services, including changing bed linens, which will facilitate the patient’s self care at home and are necessary to prevent institutionalization or re-hospitalization.

  • Performs simple vital signs.

  • Performs simple changing of non-sterile dressing changes, changing colostomy bags, emptying Foley bags, and/or non-sterile bowel and bladder hygiene care as assigned by the primary nurse case manager.

Qualifications

Minimum:

  • Must possess and maintain a current Home Health Aide certification AND Certified Nursing Assistant (CNA) certification with the state of California.

  • Must possess and maintain a current Basic Life Support (BLS) certificate.

  • Must possess and maintain a valid California driver’s license and proof of auto insurance.

  • Must provide a clean DMV printout.

Special Skills:

  • Must be able to drive a motor vehicle safely for moderate period of time.

  • Must demonstrate competency in use of Electronic Medical Record (EMR).

  • Must be able to work as a team and collaborate with clinicians.

Preferred/Desired:

  • Previous home health or hospice experience

  • Second language skills in Spanish

Pay Range

$17.85 - $24.03 /hour
