JOB SUMMARY

The Patient Safety Officer serves as the designated manager in one or more hospitals for providing leadership and expertise in the delivery of safe patient care. In doing so, the PSO will integrate information obtained from the reporting of adverse events, near misses and concerns related to clinical risk, patient complaints or other safety issues. With this information the PSO will review findings and will work collaboratively to develop responsive programs that enhance an organizational culture that supports patient safety.

ESSENTIAL FUNCTIONS OF THE ROLE

  • Reviews AE (adverse event) reports and obtains more detailed information as appropriate.

  • When adverse events cause harm to patients, the PSO will participate in choice-making regarding the implementation of an early intervention action plan.

  • Collaborates with Physician Safety Advocate and other disciplines to manage patient care processes that may create potential patient harm.

  • Is a trusted agent for clinical staff to report concerns about aspects of care that may generate patient harm.

  • Serves as chairperson or member of the Hospital's PS (Patient Safety) Committee.

  • Participates in events or situations that require Root Cause Analysis (RCA), Process Improvement (PI) or Failure Modes and Effects Analysis (FMEA).

  • Serves as the facilitator for coordinating HAC (Hospital Acquired Condition) reduction efforts.

  • Serves as the facilitator for Leap Frog submission.

  • Coordinates and works with others to insure NPSG (National Patient Safety Guidelines) are audited and compliant.

  • Based upon research of data and input from the PS Committee, develops specific programs that target reduction in adverse events or hazards. This involves recommending specific clinical process changes as appropriate.

  • With nursing managers, provides data from AE reports to support and monitor the effectiveness of safety initiative programs specifically targeted to patient populations at high risk.

  • With physician and pharmacy managers, participates in the design of policy and practice changes that will improve medication safety program.

  • Performs safety walk-arounds in patient care areas.

KEY SUCCESS FACTORS

  • Able to provide leadership and expertise in the delivery of safe patient care

  • Able to gather relevant data and review findings

  • Able to work collaboratively to develop responsive programs that support a culture of patient safety and result in a reduction of adverse events or hazards

  • Able to create a trusted environment for employees to report concerns and maintains confidentiality as appropriate

  • Able to ensure successful and compliant NPSG (National Patient Safety Guidelines) audits

  • Able to facilitate, collaborate with and participate on committees

  • COMMENTS

  • Certified Professional in Healthcare Risk Management (CPHRM), Certified Professional in Healthcare Quality (CPHQ) or Certified Professional in Patient Safety (CPPS) highly preferred

  • 3 years of experience in a risk management, quality or patient safety role

QUALIFICATIONS

  • EDUCATION - Bachelor's or 4 years of work experience above the minimum qualification

  • EXPERIENCE - 5 Years of Experience
