Trinity Health Case Manager - Case Management - PRN in Mason City, Iowa

Position Purpose:

Works to ensure the timely and seamless clinical transition of patients through medically appropriate levels of care. Uses skilled interventions and evidence based practice knowledge to collaborate (patient, physician and multidisciplinary treatment team) and achieve quality outcomes, paces care and negotiates appropriate reimbursement. The role integrates and coordinates utilization management, care facilitation and discharge planning functions. The role is heavily involved in denials for medical necessity and denials requiring concurrent review.

What you will Do:

  • Works collaboratively and maintains active communication through interdisciplinary rounds with physicians, nursing and other members of the multidisciplinary team to effect timely, appropriate patient management.

  • Serves as the primary information resource for case management staff, payors, physicians, other healthcare team members and customers (skill areas include: regulatory requirements, pathway development/implementation, ethics/healthcare law, clinical skill, advanced conflict resolution, etc.)

  • Facilitates learning experiences of healthcare team members, conducts in-services, and /or serves as a resource for pathways. Participates in teaching CM Modules.

Hours/Schedule:

PRN; as needed.

Minimum Qualifications:

Education Current and valid license to practice as Registered Nurses in Iowa, BSN or approved BA is required. Master’s Degree in Nursing or other approved course of study preferred. Case Management certification is required or willing to obtain within a year.
