Trinity Health Acute Rehab Care Coord. - Skilled Nursing - PRN in Mason City, Iowa

Position Purpose:

The Rehabilitation Care Coordinator is responsible to identify patients that have the potential to qualify for rehabilitation. The coordinator serves as a member of the rehabilitation team and oversees the process of planning, coordinating and monitoring the services and resources needed to ensure the timely and seamless transition of patients through medically appropriate levels of care. Completes the insurance prior authorizations and reauthorizations for patients on SNU and ARU.

What you will Do:

  • Conducts chart review and face-to-face assessments with patients and Medical Director to develop and document the patient's primary and back-up plan and communicates both plans to the patient/family.

  • Coordinates admission referrals from internal & external facilities to MercyOne.

  • Collaborates with physicians, rehabilitation services, pharmacist, nurses, social workers and other health professionals for care delivery and discharge planning and as a resource.

  • Develops, implements, and coordinates with Mercy Medical Center North Iowa Marketing and Planning department, state, local and federal agencies for short and long term planning

Hours/Schedule:

PRN as needed

Minimum Qualifications:

A. Education [formal]:

  • Current RN or Rehab (Physical Therapy; Occupational Therapy or Speech Pathology) licensure in the State of Iowa.

  • Minimum of a Bachelor's Degree in an approved course of study is required if candidate is an RN.

  • Minimum of Master's Degree required if candidate is a Physical Therapist, Occupational Therapist or Speech Pathologist.

B. Experience:

  • Five years clinical experience required.

  • Diverse knowledge of community services, resources and programs.

  • If RN, previous case management experience preferred.

  • If Therapist, previous acute rehab setting experience preferred. Special Skills and Competencies:

  • Computer skills, including Word, Excel, Cerner & Power Chart.

  • Clinical expertise, excellent assessment and critical thinking skills.

  • Ability to establish and maintain working relationships.

  • Excellent communication skills including public speaking, presentation and writing.

C. Completes the following mandatory courses:

  • Must provide proof of Basic Life Support (BLS Healthcare Provider, American Heart Association; or American Red Cross, Professional Rescuer-LIVE class only) via current card prior to employment or within 60 days of hire and maintains current status.

  • Completes Mandatory Reporting: Child and Dependent Adult Abuse requirement within 6 months of hire and renews this requirement every 5 years.
