Priority Health Care Medical Case Manager in Marrero, Louisiana

JOB SUMMARY:  

Assess the needs of clients and provide services to individuals diagnosed with HIV/AIDS. Provide linkage to health care, psychosocial, and other services to improve overall health of clients. 

  

DUTIES AND RESPONSIBILITIES: 

  • Prepares, manages, and evaluates an appropriate service plan for each client. 

  • Advocates and works with clients to complete goals and objectives on individual service plans within allotted time. 

  • Maintains working relationships and involvement with area hospital and clinic social workers. 

  • Access and follow-up on referrals to community resources in an appropriate amount of time. 

  • Participates in medical/educational curriculums implemented related to medical issues.          

  • (Adherence, understanding labs, etcetera) 

  • Incorporates the use of the educational curriculums with medical case managed clients. 

  • Works closely with the clients by fostering medical adherence through education and interaction with medical team. 

  • Able to complete paperwork accurately and thoroughly document in progress notes. 

  • Provides a minimum of determined billable units of service per day. 

  • Ensures client charts are complete, legible, and up to date while maintaining at least a 90% or better quarterly chart review score. 

  •  Ensures submission of required paperwork in a timely manner (log sheets, CERV, ISPs, case openings/closings/transfers, Medicaid forms, etcetera) 

  • Adheres to office protocol. (Sign in/out sheets, work hours, voicemail, staff meetings, etcetera) 

  • Interacts with clients, providers, and staff in a professional, courteous, and responsible manner. 

  • Completes 40 hours of continuing education hours/Ryan White training per year. 

  •  Performs other duties as directed by your supervisor.  

     

REQUIREMENTS: 

  • Bachelor’s degree required or master’s preferred in a human service-related field with 1 (one) year post-degree providing direct service to HIV target population. 

  • Must have sound interviewing and assessment skills and the ability to work effectively with clients of all socioeconomic and ethnic backgrounds. 

  • Ability to schedule work production to meet timelines with attention to detail. 

  • Must have a current driver’s license, reliable transportation, and proof of car insurance to travel throughout the community as required.

  • Communicates consistently with Clinic Care Manager. And other providers on operational and oversight issues.

  • None 

     

QUALIFICATIONS:

LANGUAGE SKILLS: 

  • Ability to read, write, speak, and comprehend written documents fluently. 

     

MATHEMATICAL SKILLS: 

  • Ability to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. 

     

REASONING ABILITY: 

  • Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables. 

     

CERTIFICATIONS/LICENSES/REGISTRATION: 

  • No professional certifications, licenses or registration required.  All candidates will be required to submit a criminal background check and drug screening as a part of pre-employment.
