JOB REQUIREMENTS: RN- Transitional Care Case Manager Per Diem Option 2 - 25984 DESCRIPTION/RESPONSIBILITIES: Per Diem Registered Nurses must commit to work a minimum of seventy-two (72) hours per six (6) week scheduling period as follows: The Per Diem may be required to work evening and/or night shifts, weekend hours, and holiday hours. We are seeking a Per Diem Registered Nurse (RN) Transitional Care Case Manager to: * Bridge inpatient to ambulatory care for medical, surgical, and trauma populations. * Coordinate with interdisciplinary teams to ensure remarkable healthcare. * Use clinical expertise to empower patients during post-acute hospitalization recovery. * Be an integral part of a dynamic, innovative, and fun team. Preferred - Master\'s degree in Nursing or Health Care related field. Work Experience : Minimum - Three (3) years relevant clinical nursing experience. Equivalent combination of education and experience will be considered. Preferred - Surgical, trauma, and intensive care experience. Licenses and Certifications : Minimum - RN licensed in the state of Wisconsin. CPR certification.
