Baptist Memorial Coordinator-Care RN - MG JS BlueHealth Clinic in Madison, Mississippi

Summary

Promotes patient-centered care among the healthcare team by providing coordination, collaboration and clinical expertise to meet patient needs and achieve optimal outcomes. Promotes prudent utilization of all resources. Serves as the patient's advocate by coordinating resources and services necessary to meet the patient's continuing care needs. Ensure flow of medical information between providers and points of care and close gaps where possible. Communicates and utilizes policies that support patient's fundamental right to considerate care, protects their personal dignity and respects their cultural, psychosocial and spiritual values in an effort to meet patient care needs and preferences. Reports to the Director of Care Coordination. Performs other duties as assigned.

Responsibilities

  • Promotes patient-centered care among the healthcare team by providing coordination, collaboration and clinical expertise to meet patient needs and achieve optimal outcomes.

  • Contributes to the development of a goal-directed, age appropriate, prioritized plan of care through an interdisciplinary team process that is based on diagnosis, patient needs, and expected patient outcomes.

  • Makes regular patient rounds to introduce care coordination services, communicate with physicians, case managers and other members of the healthcare team.

  • Coordinates care with members of the care team to overcome any identified barriers that inhibit medical care.

  • Acts as the patient's advocate by procuring medically indicated services and/or resources and assisting with navigating the business of healthcare.

  • Assist the physicians and their practices to close gaps and coordinate care.

  • Develops, documents, monitors and modifies a plan of care according to the patient's changing needs and MD guidance, ensuring goals are met.

  • Promotes use of evidence-based clinical protocols when warranted by patient's diagnosis.

  • Communicates with patient/family to ensure understanding of care plans and recommended treatments.

  • Serves as a resource and provides disease specific education to patients/families.

  • Initiates referrals to the appropriate areas to expedite care, treatment, and services across the care continuum.

  • Promotes health maintenance and self-care.

  • Documents pertinent information in electronic systems.

  • Serves as the patient's advocate by coordinating resources and services necessary to meet the patient's continuing care needs.

  • Collaborates with patient, family, physician, and health team members to develop a transition plan appropriate to the patient's needs and financial

  • capabilities.

  • Ensures that the patient and family understand the hospital discharge plan to promote a safe transition to the community.

  • Serves as consultant and educator to patient and family.

  • Serves as liaison for community resources.

  • Inputs pertinent data into computer system for trending.

  • Collaborates with hospital case managers and post acute providers to facilitate seamless transition of care.

  • Promotes prudent utilization of resources. Maintains current knowledge of healthcare trends and payment methodologies. Communicates with physicians regarding patient needs, including barriers to complying with the plan of care. Remains current with changes in community resources, and clinical and legal issues that affect patients and providers of care

  • Assists in Performance Improvement activities by identifying, monitoring and reporting quality measures, and participating in quality improvement teams. Monitors and reports designated measures. Participates in departmental performance/quality improvement.

  • Communicates and utilizes policies that support patient's fundamental right to considerate care, protects their personal dignity and respects their cultural, psychosocial and spiritual values in an effort to meet patient's care needs and preference.

Requirements, Preferences and Experience

Education

Preferred: Bachelor degree in nursing

Experience

Preferred: RN with three (3) years experience in acute care and ambulatory setting. Experience in a payer environment is highly desirable. Case management certification preferred or be in the process of obtaining or obtain within 1 year of employment.

Minimum: RN with professional practice experience in an acute care or ambulatory setting.

Licensure, Registration, Certification

Preferred: ACM, CCM

Minimum: RN

Special Skills

Minimum: Advanced communication, conflict management, organizational and planning skills. Advanced computer literacy skills.
