Job Description:

GRAND RE-OPENING OF OUR PARTIAL HOSPITAL PROGRAM. COME BE A PART OF BUILDING THE TEAM at Addison Gilbert Hospital!

We are looking for you to join our team. Clinicians performs a variety of direct and indirect patient care duties and responsibilities in collaboration with other nursing staff. Works effectively and collaboratively with the multi-disciplinary treatment team to enhance the smooth delivery of quality nursing care

Requirements:

Masters in Social Work or Mental Health Counseling required.

License eligible in related field(SW, LMHC, MFT.

License preferred but not required.

Essential job responsibilities:

Assumes responsibility for the clinical management of assigned patients. Conduct biopsychosocial and diagnostic assessments to appropriately determine patient needs and plan of care. Provides both individual and group based therapy for patients with various presenting problems and diagnoses. Collaborates with other clinical staff and community providers to ensure that safe and therapeutic treatment is provided for all patients. Supports the partial hospitalization mission in such a manner that there is strong and positive commitment to quality patient care.

Essential Functions/Key Responsibilities

Carry a clinical caseload and maintain the required productivity level per the Director of Ambulatory Services

Maintain appropriate records and consistently complete documentation requirements

Complete accurate diagnostic assessments, as well as develop and maintain treatment plans that are goal oriented and time limited

Regularly provide appropriate individual, group, and crisis intervention treatment for patient with various presenting problems and mental health diagnoses

Assess crisis or emergency situations and intervene immediately with appropriate clinical treatment for patients

Attend all treatment team meetings and work collaboratively with other team members

Attend all appropriate clinical and administrative departmental meetings

Support and implement administrative decisions in collaboration with treatment team and the Director of Ambulatory Services

Work with insurance to obtain approval for services provided

Utilize and/or provide supervision to enhance professional improvement and development

Maintain solid understanding of and follow regulatory agencies’ expectations and guidelines (i.e. DMH, DPH, JCAHO, payors, etc.).

Participate in any ongoing quality improvement studies that will have positive patient care outcomes.

Consistently promote professional working relationships between the hospital, referral sources and payors.
