Summary of Primary Function/General Purpose of Position

The Home Care and Hospice Lead RN is a collaborative extension of the Home Care and Hospice leadership team responsible to assist with oversight of daily operations, provide support to managers and staff members. Acts as a role model, leader, and resource to Home Care and Hospice team members and patients.

Essential Job Functions

Works collaboratively to assist Clinical Managers in planning aspects of clinical operations, day to day operations, establishing priorities, and job assignments/schedules for team members. Coordinate staff and patient schedules daily and as needed for all disciplines. Communicates with Clinical managers on a daily basis for staffing needs based on volume.

Responsible to communicate policies, assist in evaluating performance as directed by clinical manager, provide feedback, coaching and mentoring as needed. Performs on-site compliance audits as needed to proactively identify adherence to agency policy, procedures, and regulatory practices.

Coordinates and reviews the establishment and implementation of the patient’s plan of care. Monitor the delivery of patient care and the effective coordination with other services. Triages patient, family, caregiver needs as appropriate utilizing critical thinking/nursing judgement. Participates and leads interdisciplinary team meetings as requested by the clinical manager.

Participates as an active member of the agency’s quality assurance/performance improvement program. Performs chart audits and reviews as requested. Presents chart review findings as needed. Assists in identifying performance improvement areas, reporting findings to leadership, and communicates results and improvement plans to team members.

Review documentation for compliance with home care and hospice regulatory body requirements, physician orders and plan of care.

Participate and lead as directed by the clinical manager; patient case conferences, daily huddle, team meetings, education programs, annual competencies, and orientation of new hires.

Maintain a working knowledge and ensure compliance with all governmental regulations.

Works collaboratively with clinical manager and/or director to resolve patient complaints. Demonstrates excellent communication skills and customer service skills.

Performs home visits/admissions as needed to support patient needs in Home Care and Hospice.

Provides on call coverage as scheduled.

Demonstrates a positive attitude that engages team members and peers. Works with other team members to bridge communication and process workflows.

Patient Population

Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.

Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures.

Employment Qualifications

Education Qualifications: List the minimum education, training and experience required to perform the essential functions of the position

Required Minimum Education: 2 year/Associates Degree – Specialty/Major: Nursing

Preferred Education: 4 year/Bachelors Degree – Specialty/Major: Nursing

License Certification Required:

Valid Ohio Registered Nurse License

BLS – Basic Life Support

Valid Driver’s license with a driving record that passes Mercy Health driving liability insurance requirements.

Minimum Qualifications

2 years of Hospice or Home Care Nursing Case Management or Triage

Other Knowledge, Skills and Abilities Required: Home Care and Hospice regulations, leadership skills, engaging team members, innovative with new ideas, strong problem-solving skills, strong triage skills, critical thinking, excellent customer service and communications skills.

Hours

Hours per pay period: 80

Hours: 8:00am to 4:30pm

Holiday rotation, weekend call rotation..

Home Care experience preferred.
