UnitedHealth Group Medical Social Worker - Neighborhood Program in Las Vegas, Nevada

Must be willing to work at 2 additional SWM clinic locations within Las Vegas area

Preferring Acute Care, Rehab, Adult Medicine Clinic experience

Primary Responsibilities:

  • Assess patient and family psychosocial needs and develop plan of care in concert with patient, physicians, nurses and other members of the departmental team

  • Link patients/family to appropriate community resources including but not limited to, information and referral to sources of financial assistance, transportation, support groups, and other community services

  • Document patient/family status, diagnosis, treatment plan, goals, interventions, evaluation results, observations and progress in medical record

  • Serve as patient advocate and liaison with physicians, families, insurance company, community agencies and others as needed to ensure continuity of care

  • Other responsibilities as assigned

  • No option for remote work

Required Qualifications:

  • Licensed Masters prepared Social Worker (LSW or LCSW)

  • Licenses must be active and unrestricted in the State of Nevada

  • 2+ years social work experience

  • Demonstrated basic computer skills to include Word, Excel, Windows based applications

  • Ability to operate and navigate an Electronic Medical Records (touchworks, netsmart, clinical care advance, etc.)

  • Demonstrated ability to build and maintain community resource database

  • Ability to work 90% telephonic case management and 10% face-to-face with patient

Preferred Qualifications:

  • Experience in a hospital or medical setting

  • Experience in managed care

  • Case management experience

  • Working knowledge of social work theory/practice, ability to utilize a variety of diagnostic tools/techniques including DSM V State and community based programs, including rules and regulations applicable to these programs

  • Experience with advanced directives, POLST and legal regulations, including but not limited to reporting of abuse

  • Proficiency utilizing and charting in electronic medical records

Nevada Residents Only: The salary range for this role is $58,300 to $114,300 annually. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
